【摘要】：目的:比較達比加群、華法林用于心房顫動患者射頻消融術(shù)(RFCA)后的療效和安全性。方法:回顧性分析141例非瓣膜性心房顫動并擬行RFCA患者資料,按用藥的不同分為華法林組(71例)和達比加群組(70例)。華法林組患者入院前曾服華法林者入院后需停用,改為低分子肝素鈣注射液100 U/kg,皮下注射,待INR1.5時行RFCA,術(shù)前12 h停用低分子肝素鈣注射液;術(shù)中靜脈注射肝素鈣注射液100 U/kg;術(shù)后4~6 h口服華法林鈉片4.5 mg,每日1次,同時與低分子肝素橋接重疊治療至少3~5 d;術(shù)后每3 d監(jiān)測1次國際標(biāo)準化比值(INR),維持INR 2.0~3.0,至少服用華法林3個月。達比加群組患者入院后停用曾服用的抗凝藥物,改為口服達比加群酯膠囊110 mg(年齡≥70歲或體質(zhì)量60 kg)或150 mg(年齡70歲或體質(zhì)量≥60 kg),每日2次;術(shù)前24 h停用達比加群酯膠囊,術(shù)中用藥同華法林組;術(shù)后6 h口服達比加群酯膠囊,至少服用3個月。觀察兩組患者術(shù)后第1、3個月總死亡率、腦卒中(短暫性腦缺血,缺血性腦病)發(fā)生率、外周血管栓塞率及出血情況。結(jié)果:兩組患者術(shù)后第1、3個月總死亡率、腦卒中發(fā)生率、外周血管栓塞率、出血發(fā)生率比較,差異均無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:達比加群用于非瓣膜性心房顫動患者RFCA的抗凝療效和安全性均與華法林相當(dāng)。
