延長油田甘谷驛采油廠高血壓患病情況分析及其與代謝綜合征各組分關(guān)系的研究

【摘要】:【目的】1.調(diào)查和研究延長油田甘谷驛采油廠這一特殊工作人群的高血壓患病率的情況。2.通過分析不同性別、年齡人群人體測量參數(shù)及血生化代謝狀態(tài),探討各代謝綜合征各代謝指標與高血壓的相關(guān)性,為進一步干預治療提供理論依據(jù)!痉椒ā1.對甘谷驛采油廠職工采取整群抽樣的研究方法,進行問卷調(diào)查、體格檢查以及生化指標檢測。在問卷當中涉及到的調(diào)查內(nèi)容具體涉及到被檢者姓名、工種、年齡、年齡、飲食習慣、煙酒嗜好、家族病史以及既往病史等。在體格檢查的過程當中應用的是統(tǒng)一標準,具體內(nèi)容包括有身高、體重、臀圍、WC以及血壓等。在測定生化指標的過程中,先是抽取空腹靜脈血實施血糖、TC、TG、LDL-C、HDL-C、UA等相關(guān)生化指標檢測。針對無糖尿病者要做口服糖耐檢驗,而身患糖尿病者則是要做饅頭餐檢驗。2.使用Epidata 3.0構(gòu)建信息數(shù)據(jù)庫,在數(shù)據(jù)統(tǒng)計分析的過程當中,使用的是spss19.0軟件。在資料計量方面,表示為(均數(shù)±標準差),兩個樣本均數(shù)對比則應用的是t檢驗,而多樣本之間的均數(shù)對比則應用的是方差分析。患病率的對比應用的是χ2檢驗,并針對危險因素實施Logistic回歸分析,其中,α=0.05屬于檢驗水準,如P0.05代表存在顯著差異,具備統(tǒng)計學意義。3.觀察采油廠職工高血壓的總患病率,各自實施不同性別與不同年齡的患病率對比分析,針對血壓異常組和血壓正常組的相關(guān)臨床指標(FBS、PBS、TG、TC、UA、BMI、WC、臀圍、LDL-C、HDL-C、WHR、WHt R)作出對比分析,對于不同的糖代謝、尿酸代謝、血脂狀態(tài)以及體質(zhì)指數(shù)之下的高血壓患病率進行比較,探討引起采油廠職工高血壓的危險因素并對其進行相關(guān)性分析。【結(jié)果】1.共調(diào)查1290人,1288人資料納入分析。其中,男性805人,女性483人。查出高血壓患病共353人,患病率為27.41%,標化率為19.6%。其中有男性805例,高血壓患者266例,患病率33.04%,標化率為24.57%;女性483例,高血壓患者87例,其中患病率達到了18.01%,而標化率則是13.4%。男性高血壓患病率要超過女性。通過χ2檢驗分析能夠發(fā)現(xiàn),具備顯著性差異(χ2=165.65p0.001)。2.將此人群按10歲一個年齡階段分為5組,20歲~215人,高血壓患病率11.16%;30歲~257人,高血壓患病率15.95%;40歲~520人,高血壓患病率27.50%;50歲~197人,高血壓患病率43.66%;60歲~76人,高血壓患病率56.58%;70歲~23人,高血壓患病率69.57%;總的來說,高血壓的患病率有隨著年齡的增長有升高的趨勢。分析各年齡段男女血壓平均水平可以發(fā)現(xiàn),在60歲以前各組血壓平均水平都是男性高于女性,到60歲以后,男女血壓平均水平?jīng)]有顯著差異。3.高血壓患病組的血脂、血尿酸、BMI、WC、臀圍、WHR、WHt R等指標均高于血壓正常組,差異明顯(P0.05)。4.糖尿病前期的血壓患病率為44.72%,高于糖代謝正常組的18.79%,糖尿病組合并高血壓的患病率達到了48.94%,這比糖代謝正常組要高很多,具備顯著性差異(p0.05)。血脂異常組中高TG、高TC、高LDL-C、低HDL-C組中高血壓患病率分別為44.95%、56.64%、41.18%、33.53%均高于血脂代謝正常組的28.32%,差異顯著(P0.05)。高尿酸合并高血壓的患病率為68.75%,高于尿酸正常組患病率的34.84%,差異明顯(p0.05)。超重組高血壓的患病率為33.77%高于正常組的17.66%,肥胖組高血壓患病率達到了57.14%,比正常組明顯高一些,存在顯著性差異(p0.05)。且肥胖合并高血壓組的患病率高于超重組患病率,可見高血壓的患病率隨體質(zhì)指數(shù)BMI的上升有升高的趨勢,且各BMI分組之間差異有統(tǒng)計學意義(p0.05)。5.高血壓的單因素非條件Longistic回歸的分析,篩選出具有高血壓顯著相關(guān)的影響因素包括:年齡、超重和肥胖、高血糖、高血脂、以及高尿酸。6.高血壓危險因素的多因素的非條件Logistic回歸分析,把高血壓病作為因變量,其他賦值作為自變量,采用向前逐步選擇法,以a0.05水平篩選,將年齡、超重和肥胖、高血糖、高血脂、以及高尿酸帶入多因素的非條件Logistic回歸方程,研究表明,年齡、超重和肥胖、高血糖、高血脂、以及高尿酸是高血壓獨立危險的因素(P0.05),且各因素危險性排序為:高血脂(OR=2.406)、高血糖(OR=1.807)、肥胖(OR=1.740)、超重(OR=1.633)、高尿酸(OR=1.498)、年齡(OR=1.022)!窘Y(jié)論】1.目前延長油田職工高血壓患病率較高,與全國平均水平以及延安平均水平對比而言,都具有顯著性差異。高血壓的患病率隨著年齡增長有增加的趨勢,這種增長的趨勢與2002年全國調(diào)查結(jié)果相一致,高血壓的患病率表現(xiàn)為:60歲組以前男性顯著高于女性,60歲組以后男女患病率差異不大。2.在高血壓患病的危險因素當中,排在前面的主要有年齡增加、體重超標、糖脂代謝異常以及高尿酸等。
[Abstract]:[Objective] 1. to investigate and study the prevalence of hypertension in the Gangu post oil production plant of Yanchang oil field. By analyzing the parameters of human body measurement and the state of blood biochemical metabolism in different sex and age groups, the correlation between the metabolic indexes of various metabolic syndrome and hypertension was explored to provide theoretical basis for further intervention. [Methods] [Methods] 1. a questionnaire survey, physical examination and biochemical test were carried out for the staff of Gangu post oil production plant with a cluster sampling. The prevalence of blood pressure was 48.94%, which was much higher than the normal glucose metabolism group, with significant difference (P0.05). High TG, high TC, high LDL-C in dyslipidemia group, 56.64%, 41.18% and 33.53% in the low HDL-C group were respectively higher than 28.32% in the normal blood lipid metabolism group (P0.05). The prevalence rate of hyperrecombinant hypertension was 33.77% higher than that of the normal group (17.66%). The prevalence rate of obesity combined with hypertension was higher than that of the hyperrecombinant hypertension group. The prevalence of hypertension was shown to increase with the increase in the body mass index BMI, and the difference between the BMI groups was statistically significant (P0.05) for the single factor unconditional Longistic regression of.5. hypertension, and the factors that were significantly associated with hypertension included age, overweight and obesity, hyperglycemia, hyperlipidemia, and Non conditional Logistic regression analysis of the risk factors of high uricuric acid.6. hypertension, using the hypertension as the dependent variable and the other assignment as the independent variable, using the forward stepwise selection method to screen the age, overweight and obesity, hyperglycemia, hyperlipidemia, and the non conditional Logistic regression equation with high uric acid and high uric acid into the a0.05, and study the non conditional Logistic regression equation.
