NEDD4L基因與四川藏族高血壓氫氯噻嗪療效的關聯(lián)分析

【摘要】:目的:研究四川藏族輕度高血壓患者對小劑量氫氯噻嗪治療的反應和耐受性,并比較神經(jīng)前體細胞表達發(fā)育調控樣蛋白4(NEDD4L)基因SNP位點與降壓反應的關聯(lián)。方法:對符合納入排除標準的120例初發(fā)藏族輕度高血壓患者,進行問卷調查、體格檢查、血清學檢測;提取其外周血白細胞DNA進行PCR擴增,使用分子質譜法對NEDD4L基因3個SNP位點rs2288774、rs3865418和rs4149601進行分型。并對所有入選患者進行為期1個月的小劑量氫氯噻嗪(12.5mg,qd)干預。結果:該群體用藥前的平均收縮壓為153.73mmHg(1mmHg=0.133kPa),用藥后的平均收縮壓為143.52 mmHg,顯著下降10.21 mmHg;用藥前的平均舒張壓為90.94mmHg,用藥后的平均舒張壓為85.16mmHg,顯著下降5.78mmHg。40例血壓達標,達標率為36.70%。協(xié)方差分析未發(fā)現(xiàn)上述位點不同基因型攜帶者的血壓下降值有顯著差異。多元線性回歸也未發(fā)現(xiàn)上述位點與血壓下降值有顯著關聯(lián)。結論:小劑量氫氯噻嗪可在短期內顯著降低藏族輕度高血壓患者的血壓;但未發(fā)現(xiàn)以上SNP位點對降壓有顯著影響。
[Abstract]:Aim: to study the response and tolerance to low dose hydrochlorothiazide therapy in Tibetan patients with mild hypertension in Sichuan and to compare the relationship between SNP locus of developmental regulatory protein 4NEDD4L and hypotensive response in neural precursor cells. Methods: 120 primary Tibetan patients with mild hypertension who met the exclusion criteria were investigated by questionnaire, physical examination, serological examination, and peripheral blood leukocyte DNA was extracted for PCR amplification. Three SNP loci of NEDD4L gene rs2288774 rs3865418 and rs4149601 were genotyped by molecular mass spectrometry. All patients were treated with low dose hydrochlorothiazide 12.5 mg / g QD for one month. Results: the mean systolic blood pressure (SBP) of this group was 153.73mmHg1 mmHg1 (0.133 KPA), the mean systolic blood pressure (SBP) was 143.52 mmHg, the mean diastolic blood pressure (DBP) was 90.94mmHg, and the mean diastolic blood pressure (DBP) was 85.16mmHg, which decreased 5.78mmHg.40 patients' blood pressure reached the standard of 36.70Hg. Covariance analysis showed no significant difference in blood pressure among different genotype carriers. Multivariate linear regression also showed no significant correlation between the above sites and the blood pressure drop. Conclusion: low dose hydrochlorothiazide can significantly reduce blood pressure in Tibetan patients with mild hypertension in the short term, but no significant effect of SNP sites above on blood pressure is found.
