【摘要】：目的評價通竅救心油舌下給藥治療冠心病不穩(wěn)定型心絞痛的療效及安全性。方法選擇冠心病不穩(wěn)定型心絞痛患者118例,隨機分為舌下組30例、口服組29例、外搽組31例及對照組28例。在西醫(yī)標準療法的基礎(chǔ)上,舌下組舌下含服通竅救心油;口服組口服通竅救心油;外搽組用通竅救心油外搽人中穴;對照組口服硝酸異山梨酯片,心絞痛發(fā)作時可臨時增加1次用藥,各組均治療7天。治療前后觀察每周心絞痛發(fā)作次數(shù)、胸痛程度積分、胸悶程度積分,并于治療后評價患者心絞痛緩解時間療效、心電圖療效及安全性。結(jié)果治療后各組每周心絞痛發(fā)作次數(shù)、胸痛程度積分、胸悶程度積分、心絞痛緩解時間療效、心電圖療效均較治療前好轉(zhuǎn)(P0.05);治療后舌下組心絞痛緩解時間總有效率與口服組、外搽組、對照組比較差異均無統(tǒng)計學(xué)意義,但舌下組每周心絞痛發(fā)作次數(shù)、胸痛程度積分、胸悶程度積分、心絞痛緩解時間顯效率及心電圖有效率均優(yōu)于口服組與外擦組(P0.0167),與對照組比較療效相當。觀察期間,對各組患者試驗前后肝、腎功能,血、尿、便常規(guī)的異常結(jié)果進行了統(tǒng)計描述,各組均無加重。結(jié)論通竅救心油舌下給藥治療冠心病不穩(wěn)定型心絞痛療效確切且安全,在快速緩解心絞痛、減少心絞痛發(fā)作次數(shù)、減輕胸痛胸悶程度、改善心電圖心肌缺血方面與硝酸異山梨酯片療效相當,起效時間明顯優(yōu)于外搽及口服給藥。
[Abstract]:Objective to evaluate the efficacy and safety of sublingual medication in treating unstable angina pectoris of coronary heart disease. Methods 118 patients with unstable angina pectoris of coronary heart disease were randomly divided into sublingual group (n = 30), oral group (n = 29), external liniment group (n = 31) and control group (n = 28). On the basis of western medicine standard therapy, the sublingual group contained the sublingual oil for relieving the heart through orifices; the oral group took the oil for relieving the heart from the orifices; the external liniment group used the Tongqiao Jiuxin oil for the external application of the human central point; the control group took isosorbide nitrate tablets. Angina pectoris attack can be temporarily increased once, each group were treated for 7 days. The times of angina pectoris attack, the score of chest pain and the score of chest tightness were observed before and after treatment. The curative effect of angina pectoris relief time, the effect of electrocardiogram and the safety of angina pectoris were evaluated after treatment. Results after treatment, the times of angina pectoris attack, the score of chest pain, the score of chest tightness, the curative effect of angina pectoris relief time and the curative effect of electrocardiogram were all better than those before treatment, and the total effective rate of angina pectoris relief in sublingual group was higher than that in oral group. There was no significant difference between the external liniment group and the control group, but the times of angina pectoris, the degree of chest pain and the degree of chest tightness in the sublingual group were not statistically significant. The significant relief rate of angina pectoris and the effective rate of electrocardiogram were better than those of oral group and external rubbing group (P0.0167), and the curative effect was similar to that of control group. During the observation period, the abnormal results of liver, kidney function, blood, urine and stool before and after the test were statistically described. Conclusion it is effective and safe to treat unstable angina pectoris of coronary heart disease by sublingual administration of Tongqiao and Huoxin oil. It can relieve angina pectoris quickly, reduce the number of angina pectoris attacks and relieve chest pain and chest tightness. The improvement of ECG myocardial ischemia was similar to isosorbide nitrate tablets, and the time of effect was significantly better than that of external liniment and oral administration.
