【摘要】：目的探究慢性心力衰竭(CHF)患者血清脂聯(lián)素(APN)水平與心功能及左室重構(gòu)的相關(guān)性。方法選擇CHF患者112例及同期健康體檢者30例為研究對象,采用酶聯(lián)免疫吸附法(ELISA)測定血清APN水平,測量左室舒張末期內(nèi)徑(LVEDD)、左室后壁厚度(LVPWT)及室間隔厚度(IVST)、左心室質(zhì)量(LVM)、左心室質(zhì)量指數(shù)(LVMI)等心功能相關(guān)指標。采用紐約心臟病協(xié)會心功能分級標準(NYHA)對CHF患者心功能進行分級,分析血清APN水平與心功能及左室重構(gòu)的相關(guān)性。結(jié)果隨著患者心功能受損程度的增加,血清ANP逐漸升高(P=0.011);隨著患者心功能受損程度的增加,ANP、LVEDd、IVST、LVPMT、LVM與LVMI水平均顯著升高(P0.05)。CHF患者血清APN與LVEDd、IVST、LVPMT、LVM及LVMI均呈明顯正相關(guān)(r=0.511,0.615,0.411,0.441,0.531;P0.05)。結(jié)論 CHF患者APN水平越高心功能越低,且與其水平與心臟左心室重塑呈正相關(guān),可作為評價CHF患者心功能的一個重要指標。
[Abstract]:Objective to investigate the relationship between serum adiponectin (APN) level and cardiac function and left ventricular remodeling in patients with chronic heart failure (CHF). Methods Serum APN levels were measured by Elisa in 112 patients with CHF and 30 healthy controls at the same time. Left ventricular end-diastolic diameter (LVEDDN), left ventricular posterior wall thickness (LVPWT), interventricular septal thickness (IVSTT), left ventricular mass (LVM) and left ventricular mass index (LVMI) were measured. The New York Heart Association Cardiac function Classification Standard (NYHA) was used to grade the heart function of patients with CHF. The correlation between serum APN level and cardiac function and left ventricular remodeling was analyzed. Results with the increase of the degree of cardiac function impairment, the serum ANP increased gradually, and with the increase of the degree of cardiac function impairment, the levels of LVEDdT, LVPMTV, LVM and LVMI in patients with CHF were significantly higher than those in patients with P0.05. CHF. There was a significant positive correlation between serum APN and LVPMTLVM and LVMI in patients with CHF. Conclusion the higher the level of APN in CHF patients, the lower the cardiac function, and the positive correlation between APN level and left ventricular remodeling, which can be used as an important index to evaluate cardiac function in CHF patients.
