【摘要】：目的探索治療老年慢性心力衰竭(CHF)合并抑郁障礙時在常規(guī)內(nèi)科治療基礎(chǔ)上加用艾司西酞普蘭對認(rèn)知功能和心功能的近期影響。方法選取2014年10月至2015年10月重慶市第九人民醫(yī)院收治的97例CHF合并抑郁障礙患者,隨機(jī)分為抗抑郁組和對照組。抗抑郁組在常規(guī)內(nèi)科治療基礎(chǔ)上加用艾司西酞普蘭,對照組則加用安慰劑。治療前和治療6周末采用24項漢密爾頓抑郁量表(HAMD-24)、14項漢密爾頓焦慮量表(HAMA-14)、蒙特利爾認(rèn)知評估量表(MoCA)評估患者的抑郁、焦慮程度和認(rèn)知功能,并檢測患者血漿N-末端腦利鈉肽原(NT-proBNP)水平、左心室舒張末內(nèi)徑(LVEDD)及左心室射血分?jǐn)?shù)(LVEF)。結(jié)果治療6周末,抗抑郁組HAMD-24和HAMA-14得分、血漿NT-ProBNP水平低于對照組,注意得分和LVEF高于對照組,差異均有統(tǒng)計學(xué)意義(P0.05);其余各項觀察指標(biāo)比較,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論艾司西酞普蘭治療老年CHF合并抑郁障礙不僅能緩解患者的焦慮抑郁情緒,還能促進(jìn)患者心功能和注意的改善。
[Abstract]:Objective to explore the short-term effects of Escitalopram on cognitive function and cardiac function in elderly patients with chronic heart failure (CHF) complicated with depressive disorder on the basis of routine medical treatment. Methods from October 2014 to October 2015, 97 patients with CHF complicated with depressive disorder in Chongqing Ninth people's Hospital were randomly divided into antidepressant group and control group. The antidepressant group was treated with estalopram on the basis of routine medical treatment and the control group with placebo. Before and after 6 weeks of treatment, 24 items of Hamilton Depression scale (Hamd) and 14 items of Hamilton anxiety scale (Hamilton anxiety scale) and Montreal Cognitive Assessment scale (MOCA) were used to evaluate the depression, anxiety and cognitive function of the patients. Plasma NT-proBNPs, left ventricular end-diastolic diameter (LVEDDD) and left ventricular ejection fraction (LVEF) were measured. Results at the end of 6 weeks, the scores of HAMD-24 and HAMA-14, the level of plasma NT-ProBNP in antidepressant group were lower than those in the control group, the attention score and LVEF were higher than those in the control group, the difference was statistically significant (P 0.05), but there was no significant difference in other observation indexes (P 0.05). Conclusion Escitalopram can not only relieve anxiety and depression, but also improve cardiac function and attention in elderly patients with CHF combined with depression.
