【摘要】：目的探討65歲以上老年人不同高血壓類(lèi)型是否存在性別差異。方法 65~88歲高血壓患者125例,其中男81例,女44例,行24 h動(dòng)態(tài)血壓監(jiān)測(cè)后,按表示晝夜節(jié)律變化的血壓變異性(BPV)分4型,反杓型:BPV0;非杓型:10%BPV0;杓型:20%BPV≥10%;超杓型:BPV≥20%,并各自按老年男性與老年女性進(jìn)行分組,監(jiān)測(cè)參數(shù)包括24-SBP、24-DBP、D-MAP、N-MAP、24-BPVS、D-BPVS、N-BPVS、24-BPVD、DBPVD、N-BPVD、D-SBP-P、D-SBP-V、D-DBP-P、D-DBP-V、N-SBP-P、N-SBP-V、N-DBP-P、NDBP-V。結(jié)果 (1)正常的杓型組占比21.6%、非正常血壓組(反杓型+非杓型+超杓型)占比78.4%;(2)反杓型老年女性除D-SBP-P外,其余日間與夜間血壓峰值與谷值均較老年男性低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);非杓型組老年女性在N-SBP-V、N-DBP-P、N-DBP-V較老年男性低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);(3)反杓型組在24-SBP、24-DBP、D-MAP、N-MAP與非杓型組24-SBP、24-DBP老年女性較老年男性組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);(4)反杓組24-BPVS、D-BPVS、24-BPVD、D-BPVD,非杓型組N-BPVS、24-BPVD、N-BPVD,杓型組24-BPVS、N-BPVS、24-BPVD、N-BPVD指標(biāo)老年女性較老年男性大,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論非正常杓型高血壓占比80%左右,對(duì)于65歲以上的高血壓患者應(yīng)進(jìn)行24 h動(dòng)態(tài)血壓分型,反杓型、非杓型、杓型高血壓患者老年女性的血壓變異性大于老年男性,有性別差異。
[Abstract]:Objective to investigate whether there are gender differences in different types of hypertension in the elderly over 65 years of age. Methods 125 cases of 65~88 year old hypertension patients, including 81 men and 44 women, were divided into 4 type of blood pressure variability (BPV) according to circadian rhythm, BPV0, non dipper type, 10%BPV0, Dipper type, 20%BPV > 10%, BPV more than 20%, after 24 h ambulatory blood pressure monitoring. The monitoring parameters including 24-SBP, 24-DBP, D-MAP, N-MAP, 24-BPVS, D-BPVS, N-BPVS, 24-BPVD, DBPVD, N-BPVD, D-SBP-P, D-SBP-V, N-BPVD, D-SBP-P, 21.6%, and non normal blood pressure group (anti dipper type + non dipper + Super dipper) accounted for 78. (2) (2) the elderly women of the anti dipper type were lower than the old men except D-SBP-P, and the difference was statistically significant (P0.05). The elderly women in the non dipper group were lower in N-SBP-V, N-DBP-P, N-DBP-V than in the old men (P0.05); (3) the anti dipper group was in 24-SBP, 24-DBP, D-MAP, N-MAP and non dipper group 24. -SBP, 24-DBP elderly women were lower than the elderly male group (P0.05), and (4) anti dipper group 24-BPVS, D-BPVS, 24-BPVD, D-BPVD, non dipper group N-BPVS, 24-BPVD, N-BPVD, 24-BPVS in the dipper group, N-BPVS, the difference has statistical significance. Conclusion abnormal dipper type hypertension accounted for 80%. The hypertension patients over 65 years of age should be divided into 24 h dynamic blood pressure typing, anti dipper type, non dipper type, and dipper type hypertension patients with older women's blood pressure variability than older men, there is a gender difference.
