【摘要】：目的:分析2013年廣西冠心病的經(jīng)濟(jì)負(fù)擔(dān)及其住院費(fèi)用的影響因素,為衛(wèi)生行政部門制定相關(guān)衛(wèi)生政策提供依據(jù)。方法:(1)收集2013年廣西冠心病住院患者的病案信息,測(cè)算冠心病住院費(fèi)用,采用多重線性回歸分析住院費(fèi)用的影響因素。(2)通過冠心病經(jīng)濟(jì)負(fù)擔(dān)問卷調(diào)查,測(cè)算2013年冠心病的門診費(fèi)用、直接非醫(yī)療經(jīng)濟(jì)負(fù)擔(dān)、親屬間接經(jīng)濟(jì)負(fù)擔(dān)。(3)通過全球疾病負(fù)擔(dān)(GBD)獲取2013年中國(guó)冠心病性別年齡別YLD、YLL、DALY和2013年中國(guó)性別年齡別期望壽命,運(yùn)用傷殘調(diào)整生命年(DALY)結(jié)合人力資本法測(cè)算患者間接經(jīng)濟(jì)負(fù)擔(dān)。結(jié)果:(1)2013年廣西冠心病住院患者共31107例,男性18906例,女性12201例,男女比例為1.55:1,平均年齡67.44±12.57歲,平均住院天數(shù)為6天,手術(shù)治療的患者占29.39%。(2)2013年廣西冠心病患者總住院費(fèi)用4.06億元,例均住院費(fèi)用為6150.73元,其中手術(shù)患者例均住院費(fèi)用為1.74萬元,非手術(shù)患者的為4800.92元。以醫(yī)用材料費(fèi)、西藥費(fèi)所占比重較大,分別為33.98%、20.51%。(3)2013年廣西冠心病總經(jīng)濟(jì)負(fù)擔(dān)為9.60億元,住院費(fèi)用、門診費(fèi)用、直接非醫(yī)療經(jīng)濟(jì)負(fù)擔(dān)、患者間接經(jīng)濟(jì)負(fù)擔(dān)、親屬間接經(jīng)濟(jì)負(fù)擔(dān)分別為4.06億元、0.09億元、0.03億元、5.26億元、0.16億元。(4)多重線性回歸結(jié)果顯示,性別、年齡、婚姻狀況、付費(fèi)方式、手術(shù)、住院天數(shù)、合并其他疾病是冠心病住院費(fèi)用的影響因素。手術(shù)和住院天數(shù)是主要的影響因素。結(jié)論:(1)2013年廣西冠心病經(jīng)濟(jì)負(fù)擔(dān)相對(duì)沉重。(2)2013年廣西冠心病的例均住院費(fèi)用相對(duì)其他病種處于中等水平;冠心病手術(shù)患者的例均住院費(fèi)用高于非手術(shù)患者。(3)手術(shù)和住院天數(shù)是冠心病住院費(fèi)用的主要影響因素。
[Abstract]:Objective: to analyze the economic burden of coronary heart disease and the influencing factors of hospitalization cost in Guangxi in 2013 so as to provide basis for health administration department to formulate relevant health policies. Methods: 1) collect the medical record information of inpatients with coronary heart disease in Guangxi in 2013, calculate the hospitalization cost of coronary heart disease, and use multiple linear regression analysis to analyze the influencing factors of hospitalization expenses. The outpatient cost of coronary heart disease in 2013, the direct non-medical financial burden, the indirect economic burden of relatives, and the global disease burden GBD) were used to obtain the gender and age specific life expectancy of Chinese coronary heart disease in 2013 and 2013, respectively. The indirect economic burden of patients was calculated by using DALY and human capital method. Results in 2013, there were 31107 inpatients with coronary heart disease in Guangxi, 18906 males and 12201 females. The ratio of male to female was 1.55: 1, the average age was 67.44 鹵12.57 years old, the average hospitalization time was 6 days. The total hospitalization cost of coronary heart disease patients in Guangxi was 406 million yuan in 2013. The average hospitalization cost was 6150.73 yuan, including 17400 yuan for the patients with operation and 4800.92 yuan for the patients without operation. In 2013, the total economic burden of coronary heart disease in Guangxi was 960 million yuan, hospital expenses, outpatient expenses, direct non-medical economic burdens, indirect economic burdens of patients, and so on. The indirect economic burden of relatives was 406 million yuan, 9 million yuan, 3 million yuan, 526 million yuan, 16 million yuan respectively) multiple linear regression results showed that sex, age, marital status, payment method, operation, hospital stay, Associated with other diseases is the impact of coronary heart disease hospitalization costs. Operation and hospital stay were the main influencing factors. Conclusion (1) the economic burden of coronary heart disease in Guangxi is relatively heavy in 2013. (2) the average hospitalization cost of coronary heart disease in Guangxi in 2013 is at the medium level compared with other diseases. The average hospitalization cost of the patients with coronary heart disease was higher than that of the patients without coronary heart disease (3) the operation and hospitalization days were the main factors influencing the hospitalization cost of coronary heart disease.
