老年人群診室、隨診間、動態(tài)收縮壓與肱踝脈搏波傳導(dǎo)速度的相關(guān)性

【摘要】:目的分析診室、隨診間、24小時動態(tài)收縮壓(SBP)與肱踝脈搏波傳導(dǎo)速度(ba PWV)的關(guān)聯(lián)。方法自2006年6月-2007年10月,由開灤總醫(yī)院、開灤林西醫(yī)院、開灤趙各莊醫(yī)院等共計11家醫(yī)院對開灤集團(tuán)在職及離退休員工進(jìn)行第1查體,體檢過程中包括問卷調(diào)查以及體格檢查和甘油三酯、超敏C反應(yīng)蛋白、低密度脂蛋白等生化指標(biāo)的檢測。此后分別于2008年07月-2009年10月、2010年07月-2011年10月由醫(yī)務(wù)人員按第1次查體的時間順序在相同地點(diǎn)對同一人群分進(jìn)行第2、3次查體。在參加開灤研究第3次查體的人群中,采用整群抽樣的方法抽取2464例年齡≥60歲的開灤集團(tuán)離退休員工進(jìn)行第3+次查體。體檢內(nèi)容包括測量診室血壓、24小時動態(tài)血壓監(jiān)測(24h ABPM)及ba PWV檢查。依據(jù)診室140 mm Hg和≥140 mm Hg將總?cè)巳悍譃閮山M,比較兩組間基線及ba PWV情況;采用多元逐步線性回歸分析不同類型SBP對ba PWV的影響;采用多因素Logistic回歸分析建立影響ba PWV的回歸模型,并比較不同類型SBP與baPWV的相關(guān)性大小。結(jié)果1共納入907例資料完整的研究對象進(jìn)行統(tǒng)計分析。2高SBP組的ba PWV值及ba PWV≥1400 cm/s的檢出率均高于低SBP組(P0.01)。3多元逐步線性回歸分析顯示:將診室、隨診間、動態(tài)SBP同時代入線性回歸方程,校正年齡、性別等其他混雜因素,診室SBP與ba PWV的關(guān)系相關(guān)性較強(qiáng),優(yōu)于動態(tài)SBP和隨診間SBP。4在多因素Logistic回歸分析中,將診室、隨診間、動態(tài)SBP同時代入方程,并校正其他混雜因素,結(jié)果顯示診室、隨診間、動態(tài)SBP均是ba PWV增加的危險因素,其OR值分別為8.39(P0.05)、1.49(P=0.481)、3.32(P0.05)。結(jié)論診室SBP與ba PWV的相關(guān)性最強(qiáng),優(yōu)于隨診間SBP和動態(tài)SBP。
[Abstract]:Objective to analyze the relationship between 24 hour systolic blood pressure (SBP) and brachial ankle pulse wave velocity (BWV). Methods from June 2006 to October 2007, a total of 11 hospitals including Kailuan General Hospital, Kailuan Linxi Hospital and Zhaogezhuang Hospital of Kailuan Group carried out the first physical examination of the working and retired employees of Kailuan Group. The physical examination included questionnaire, physical examination, triglyceride, hypersensitive C-reactive protein, low density lipoprotein and so on. From July 2008 to October 2009, from July 2010 to October 2011, the medical staff conducted the second third physical examination of the same population in the same place according to the time sequence of the first physical examination. Among the people who took part in the third physical examination of Kailuan study, 2464 retired employees of Kailuan Group were selected by cluster sampling method for the third physical examination.
