【摘要】：目的:探究合并快靜息心率的高血壓患者的臨床特征及其常見臨床合并癥情況。方法:本研究回顧性分析了2000年山東青島港研究和2005年河南信陽研究納入的靜息心電圖資料完整的18歲以上社區(qū)人群,包括高血壓與非高血壓患者。收集患者的臨床合并癥(包括糖尿病、冠心病、高脂血癥、腦卒中等)、實驗室檢查結(jié)果、心電圖靜息心率(RHR)、體重指數(shù)(BMI)、腰臀比以及診室血壓。結(jié)果:納入受試者18 183例,61.6%為男性,平均年齡(45.2±12.7)歲,高血壓患者6 763例。高血壓患者的靜息心率明顯高于正常血壓人群[(73.5±11.6)次/min vs(70.6±9.6)次/min,P0.001],合并快靜息心率的患者比例顯著高于非高血壓者(14.5%vs 6.4%,P0.001)。在高血壓患者中,與正常靜息心率的患者相比,合并快靜息心率的患者年齡更大[(51.8±11.2)歲vs(53.9±12.2)歲,P0.001),體重指數(shù)更低([26.4±3.4)kg/m~2 vs(25.8±3.6)kg/m~2,P0.001],3級高血壓患者比例更高(59.0%vs 68.2%,P0.001),空腹血糖[(5.6±1.5)mmol/L vs(6.0±2.4)mmol/L,P0.001]、甘油三酯([1.7±1.3)mmol/L vs(2.0±1.8)mmol/L,P0.001]及低密度脂蛋白膽固醇水平更高([3.1±0.8)mmol/L vs(3.2±0.9)mmol/L,P=0.001)],合并糖尿病(3.9%vs 6.6%,P=0.007)和腦卒中(8.3%vs 11.1%,P=0.005)的比例高。多因素回歸分析校正傳統(tǒng)危險因素后發(fā)現(xiàn),快靜息心率與高血壓患者的腦卒中風險呈正相關(guān)(OR=1.306,95%CI:1.021~1.671)。結(jié)論:高血壓人群中快靜息心率比例顯著高于正常血壓人群。合并快靜息心率的高血壓患者年齡更大,體重指數(shù)更低,多為3級高血壓患者,且多合并糖尿病、腦卒中。快靜息心率增加高血壓患者的腦卒中風險。
[Abstract]:Objective: to investigate the clinical features and common clinical complications of hypertensive patients with fast resting heart rate. Methods: this study retrospectively analyzed the community population over 18 years old with complete resting ECG data, including hypertension and non-hypertension, which were included in the study of Qingdao Port, Shandong Province in 2000 and Xinyang study in Henan Province in 2005. Clinical complications (including diabetes, coronary heart disease, hyperlipidemia, stroke, etc.), laboratory findings, resting heart rate (RHRG), body mass index (BMI), waist-to-hip ratio (WHR), and laboratory blood pressure were collected. Results: 18 183 subjects were male (61.6%), with an average age of 45.2 鹵12.7 years, and 6 763 patients with hypertension.
