【摘要】：目的探討超聲檢查對血友病患者膝關(guān)節(jié)狀況評估的應(yīng)用價值。方法收集44例血友病患者中70個膝關(guān)節(jié)行超聲檢查,其中11例小于18歲患者中16個膝關(guān)節(jié)完成磁共振成像(MRI)檢查,同時對膝關(guān)節(jié)按照2.1版血友病關(guān)節(jié)健康評估表(HJHS)評價其健康狀況,分析超聲評分與年齡及出血次數(shù)的相關(guān)性,以及與MRI評分和HJHS評分之間的關(guān)系。結(jié)果血友病關(guān)節(jié)病變嚴重程度與血友病的嚴重程度無相關(guān)性(F=0.013,P=0.987),隨著年齡增加、出血次數(shù)增多,膝關(guān)節(jié)病變加重。關(guān)節(jié)病變早期的滑膜增厚及軟骨改變的檢出,超聲及MRI檢查無差異(P0.05)。超聲評分、MRI評分、HJHS之間存在明顯相關(guān)性(P0.05)。結(jié)論超聲對于血友病性關(guān)節(jié)病的早期診斷有重要意義。
[Abstract]:Objective to evaluate the value of ultrasonic examination in evaluation of knee joint status in hemophilia patients. Methods Sixty knees of 44 hemophilia patients were examined by ultrasonography, 16 of them (11 patients younger than 18 years old) completed Mr imaging (MRI). At the same time, the health status of knee joint was evaluated according to HJHS2.1 version hemophilia joint health assessment form. The relationship between ultrasound score and age, bleeding times, MRI score and HJHS score was analyzed. Results there was no correlation between the severity of hemophilia joint lesion and the severity of hemophilia. Synovial thickening and cartilage change were detected in the early stage of articular lesions. There was no difference in ultrasonography and MRI. There was a significant correlation between MRI score and HJHS (P 0.05). Conclusion Ultrasound is important for the early diagnosis of hemophilia arthrosis.
