【摘要】：目的:回顧性分析參附注射液結(jié)合常規(guī)西醫(yī)治療與單獨(dú)應(yīng)用常規(guī)西醫(yī)治療全心衰的患者臨床效果,評(píng)價(jià)參附注射液聯(lián)合西醫(yī)治療的療效,進(jìn)一步為治療全心衰提供新的臨床思路。材料與方法:1.臨床資料:納入2016年1月-2016年8月期間在遼寧省人民醫(yī)院心臟中心臨床診斷為全心衰住院患者共50例,研究組29例,對(duì)照組21例。2.研究組在常規(guī)心衰治療基礎(chǔ)上加用參附注射液80ml加入5%葡萄糖注射液100ml靜脈滴注10天,對(duì)照組應(yīng)用常規(guī)強(qiáng)心、利尿、擴(kuò)血管等常規(guī)心衰治療。比較治療前與治療后1個(gè)月后左室舒張末內(nèi)徑(LVEDD),左室射血分?jǐn)?shù),肺動(dòng)脈壓力;6分鐘步行實(shí)驗(yàn);N末端B型利鈉肽(NT-pro BNP);心衰中醫(yī)臨床證候量化評(píng)分;西醫(yī)心功能以及治療過程中肝腎功損害等的不良反應(yīng),以評(píng)定臨床治療效果。結(jié)果:1.一般資料:兩組數(shù)據(jù)一般資料無統(tǒng)計(jì)學(xué)差異(p0.05)。2.西醫(yī)心功能評(píng)估:研究組可以明顯改善患者心力衰竭癥狀(χ~2=6.948,p=0.0310.05)。3.中醫(yī)臨床證候分級(jí)量化積分及6分鐘步行實(shí)驗(yàn)評(píng)價(jià)研究組有統(tǒng)計(jì)學(xué)意義(t=2.487,p=0.0170.05;χ~2=11.06,p=0.0090.05)。4.兩組左室射血分?jǐn)?shù)、左室舒張末內(nèi)徑、肺動(dòng)脈高壓、NT-pro BNP比較有統(tǒng)計(jì)學(xué)意義,顯示結(jié)合參附注射液治療會(huì)改善患者相關(guān)指標(biāo)(t=3.249,p=0.000.05;t=5.898,p=0.0000.05;t=3.198,p=0.0030.05;t=2.827,p=0.0070.05)。5、未見明顯不良反應(yīng)。結(jié)論:1.參附注射液聯(lián)合常規(guī)西醫(yī)治療可以改善全心衰患者的心功能,并可以有效改善患者心悸、乏力、氣喘、惡心、浮腫等等中醫(yī)證候。2.參附注射液聯(lián)合常規(guī)西醫(yī)治療全心衰的療效優(yōu)于單純應(yīng)用西醫(yī)常規(guī)治療。3.未發(fā)生明顯的藥物不良反應(yīng),臨床應(yīng)用較為安全。
[Abstract]:Objective: to analyze retrospectively the clinical effect of Shenfu injection combined with conventional western medicine and conventional western medicine in treating total heart failure, and to evaluate the curative effect of Shenfu injection combined with western medicine. To further provide a new clinical thinking for the treatment of total heart failure. Materials and methods: 1. Clinical data: from January 2016 to August 2016, a total of 50 patients with total heart failure were diagnosed in the heart center of Liaoning Provincial people's Hospital, 29 patients in the study group and 21 cases in the control group. The study group was treated with Shenfu injection 80ml and 5% glucose injection 100ml for 10 days on the basis of routine heart failure treatment, while the control group was treated with routine heart failure, such as strengthening heart, diuretic and vasodilating. The left ventricular end-diastolic diameter (LVEDDG), left ventricular ejection fraction (LVEF), pulmonary artery pressure (PAP), N-terminal B-type natriuretic peptide (NT-pro BNPN) were compared before treatment and 1 month after treatment. In order to evaluate the clinical therapeutic effect, the adverse reactions of the heart function and the damage of liver and kidney in the course of treatment were evaluated. The result is 1: 1. General data: there was no statistical difference between the two groups of data. Evaluation of cardiac function in western medicine: the study group could obviously improve the symptoms of heart failure in patients (蠂 ~ (2 / 2) 6.948) 0.0310.05 ~ (.3). The quantitative score of clinical syndromes of TCM and the evaluation of 6-minute walking experiment in the study group had statistical significance (P 0.0170.05) and 蠂 ~ (2 +) (11.06) / (0.0090.05) ~ (.4). Left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEF) and NT-pro BNP were significantly different between the two groups. The results showed that treatment with Shenfu injection could improve the related indexes of patients. Conclusion 1. Shenfu injection combined with routine western medicine treatment can improve the heart function of patients with total heart failure, and can effectively improve palpitation, fatigue, asthma, nausea, swelling and other TCM syndromes. 2. The therapeutic effect of Shenfu injection combined with conventional western medicine on total heart failure is better than that of western medicine alone. No obvious adverse drug reactions occurred, and clinical application was relatively safe.
