H型高血壓患者頸動(dòng)脈內(nèi)中膜厚度與血清Hcy、Cyst-c、UA的相關(guān)性研究

【摘要】:目的:探討血清同型半胱氨酸、胱抑素-c、尿酸水平與H型高血壓患者頸動(dòng)脈內(nèi)中膜厚度的相關(guān)性,為動(dòng)脈粥樣硬化疾病的早期干預(yù)及治療提供理論依據(jù)。方法:根據(jù)入選、排除標(biāo)準(zhǔn),選取2014年12月-2015年10月青海大學(xué)附屬醫(yī)院入院治療的H型高血壓3級(jí)患者101例,男53例、女48例。詳細(xì)記錄患者病史,測(cè)量血壓,身高、體重,計(jì)算體重指數(shù),測(cè)定血漿同型半胱氨酸、胱抑素-c、尿酸,空腹血糖、甘油三酯、總膽固醇、高密度脂蛋白、低密度脂蛋白(LDL-C)、載脂蛋白A、載脂蛋白B等指標(biāo),并行頸動(dòng)脈彩超檢查,根據(jù)頸動(dòng)脈內(nèi)中膜厚度分三組:頸動(dòng)脈內(nèi)中膜正常組34例、頸動(dòng)脈硬化組31例、頸動(dòng)脈斑塊組36例,并將正常組設(shè)置為對(duì)照組。檢測(cè)并比較3組的血清指標(biāo),分析其與頸動(dòng)脈內(nèi)中膜厚度的關(guān)系。利用SPSS20.0進(jìn)行統(tǒng)計(jì)分析。結(jié)果:1、三組間性別、年齡、吸煙史、飲酒史、病史、FBG、TG、TC、LDL-C、APOA、APOB、BMI等指標(biāo)比較差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。2、三組間Hcy、Cyst-c、UA、HDL-C的比較分別為:Hcy水平比較:斑塊組對(duì)照組,硬化組對(duì)照組(P0.05);Cyst-c水平比較均為:斑塊組對(duì)照組(P0.05);UA水平比較:斑塊組對(duì)照組(P0.05);HDL-C水平比較:斑塊組對(duì)照組,硬化組對(duì)照組(P0.05)。3、Hcy、Cyst-C、UA與cIMT的相關(guān)性分析:Hcy、Cyst-C、UA與c IMT呈正相關(guān)(0.334、0.317、0.442,P值分別為0.001、0.001、0.000);HDL-C與cIMT呈負(fù)相關(guān)(r=-0.293,P=0.003)。結(jié)論:血清Hcy、Cyst-c、UA是AS的危險(xiǎn)因素,HDL-C是AS的保護(hù)性因子,監(jiān)測(cè)血清中各項(xiàng)指標(biāo)水平的變化,對(duì)AS疾病的早期干預(yù)及治療具有一定的參考價(jià)值。
[Abstract]:Objective: to investigate the correlation between serum homocysteine, cystatin-cand uric acid levels and carotid intima-media thickness in patients with H type hypertension, and to provide theoretical basis for early intervention and treatment of atherosclerosis. Methods: according to the inclusion and exclusion criteria, 101 H type hypertension grade 3 patients, 53 males and 48 females, were admitted to the affiliated Hospital of Qinghai University from December 2014 to October 2015. Record the patient's history, measure blood pressure, height, weight, calculate body mass index, measure plasma homocysteine, cystatin, uric acid, fasting blood glucose, triglyceride, total cholesterol, high density lipoprotein, measure plasma homocysteine, cystatin, uric acid, fasting blood glucose, triglyceride, total cholesterol, high density lipoprotein, Low density lipoprotein (LDL-C), apolipoprotein A (apo A) and apolipoprotein B (apo B) were divided into three groups according to carotid intima-media thickness: 34 cases in normal carotid artery group, 31 cases in carotid atherosclerosis group, 36 cases in carotid plaque group, 36 cases in carotid artery plaque group, 34 cases in normal carotid artery group, 31 cases in carotid atherosclerosis group and 36 cases in carotid plaque group. The normal group was set as the control group. The serum indexes of the three groups were measured and compared, and the relationship between them and carotid intima media thickness was analyzed. SPSS20.0 was used for statistical analysis. Results there were no significant differences in sex, age, smoking history, drinking history, history of FBGG, TGN, LDL-C and APOAA APOBN BMI between the three groups (P 0.05. 2). There was no significant difference in HDL-C between the three groups. The comparison of HDL-C between the three groups was compared with that of the control group (P < 0.05). The comparison of HDL-C between the three groups was as follows: compared with the control group, there was no significant difference in HDL-C between the three groups. The comparison of the levels of Cyst-c in the sclerosing group and the control group were as follows: the comparison of the levels of P0.05U and UA in the plaque group, the comparison of the HDL-C level in the control group of the plaque group and the control group, the comparison of the levels of HDL-C in the plaque group and the control group. Correlation between Cyst-Con UA and cIMT in the sclerosing control group the positive correlation between Cyst-Con UA and c IMT was 0.334U 0.3170.442 (P = 0.001 0. 001D 0.000) and negative correlation between HDL-C and cIMT was r-0.293P = 0. 003P = 0. 003P = 0. 001 ~ 0. 000 P = 0. 003a, P = 0. 001C = 0. 003a, P = 0. 003a, P = 0. 003a, P = 0. 003a respectively. Conclusion: the serum Hcystatin Cyst-ctrocardia is the risk factor of as. HDL-C is the protective factor of as. Monitoring the changes of serum indexes is valuable for the early intervention and treatment of as disease.
