【摘要】：研究目的將靜態(tài)文本形式的臨床指南按照合適的指南表達(dá)模型進(jìn)行建模形成計(jì)算機(jī)可理解的形式,將本體理論和知識(shí)庫技術(shù)結(jié)合起來,把文本形式臨床指南轉(zhuǎn)換成方便推理的知識(shí)庫,有助于提高臨床指南的使用效率,減少臨床實(shí)踐活動(dòng)中醫(yī)生的診療失誤。以此為基礎(chǔ)也可以作為高血壓臨床決策支持系統(tǒng)的重要模塊,降低設(shè)計(jì)構(gòu)建的難度。研究?jī)?nèi)容1.醫(yī)學(xué)知識(shí)庫構(gòu)建相關(guān)理論對(duì)比分析通過對(duì)醫(yī)學(xué)知識(shí)庫構(gòu)建的相關(guān)技術(shù)總結(jié)分析,找到適合高血壓醫(yī)學(xué)知識(shí)庫設(shè)計(jì)與構(gòu)建的模型與技術(shù)。如本體構(gòu)建的語言和工具及本體推理機(jī)等,分析其各自的優(yōu)缺點(diǎn)和應(yīng)用范圍,確定適用于本文研究的高血壓臨床指南應(yīng)用的語言及模型。2.高血壓邏輯本體庫構(gòu)建研究在本文中將高血壓相關(guān)的病理知識(shí)提取出來加以整合,將本體的思想與臨床指南知識(shí)相結(jié)合,對(duì)臨床指南知識(shí)進(jìn)行結(jié)構(gòu)化和非結(jié)構(gòu)化的表示,提取出的相關(guān)高血壓概念建立領(lǐng)域描述邏輯本體,形成由概念、屬性、實(shí)例以及相關(guān)概念之間關(guān)系組成的本體庫。3.高血壓醫(yī)學(xué)知識(shí)表達(dá)模型設(shè)計(jì)研究結(jié)合臨床醫(yī)生工作流程,將高血壓臨床指南分解成流程圖的方式。如高血壓篩查、檢查評(píng)估、危險(xiǎn)分層、藥物與非藥物治療、隨訪管理等。根據(jù)流程圖化的高血壓臨床指南建立醫(yī)學(xué)知識(shí)表達(dá)模型。4.高血壓規(guī)則庫整合研究利用推理引擎,將醫(yī)學(xué)知識(shí)表達(dá)模型轉(zhuǎn)換成符合計(jì)算機(jī)可推理要求的一系列規(guī)則,構(gòu)建規(guī)則庫,完成高血壓醫(yī)學(xué)知識(shí)庫的構(gòu)建。如高血壓診斷規(guī)則、用藥規(guī)則等。研究方法1.通過文獻(xiàn)與網(wǎng)站調(diào)研,梳理本體、臨床指南知識(shí)表達(dá)模型等相關(guān)理論和國(guó)內(nèi)外經(jīng)驗(yàn)做法,總結(jié)出適合高血壓臨床指南知識(shí)庫的構(gòu)建流程。2.本文采用本體開發(fā)工具Protege 4.3構(gòu)建高血壓臨床指南本體,并參考SAGE模型進(jìn)行臨床指南知識(shí)表達(dá),并將指南中的規(guī)則以流程圖的方式展示,在Protege 4.3中實(shí)現(xiàn)本體構(gòu)建、知識(shí)表達(dá)以及本體推理過程。研究結(jié)果1.主要借鑒七步法的思路,改進(jìn)國(guó)際通過疾病本體模型,并參考MeSH、CMeSH主題詞表的結(jié)構(gòu),對(duì)高血壓臨床指南進(jìn)行本體構(gòu)建。2.在對(duì)比傳統(tǒng)的臨床指南知識(shí)表達(dá)模型后,選擇SAGE模型構(gòu)建高血壓臨床指南知識(shí)表達(dá)模型,提高了臨床指南知識(shí)表達(dá)的靈活性和科學(xué)性。3.將高血壓臨床指南中的診斷方法使用語義網(wǎng)規(guī)則語言SWRL構(gòu)建成高血壓SWRL診斷規(guī)則庫,為高血壓臨床指南本體的推理提供了依據(jù)。4.使用Jess作為推理機(jī),結(jié)合高血壓臨床指南本體和SWRL診斷規(guī)則庫實(shí)現(xiàn)了高血壓知識(shí)庫的診斷推理,并且用一組測(cè)試實(shí)例驗(yàn)證了診斷推理的準(zhǔn)確性和科學(xué)性�？偨Y(jié)本文針對(duì)臨床指南在臨床應(yīng)用中出現(xiàn)的一些障礙,提出了一種全新的臨床指南數(shù)字化方法解決臨床指南發(fā)展過程中的瓶頸。高血壓疾病作為威脅人類健康的一種常見病,將高血壓臨床指南和本體研究結(jié)合起來構(gòu)建高血壓臨床指南知識(shí)庫不僅可以提高指南在臨床中的利用率,也可以被臨床決策支持系統(tǒng)直接使用。同時(shí),高血壓臨床指南知識(shí)庫的構(gòu)建方法也適用于其它疾病臨床指南知識(shí)庫的構(gòu)建,為臨床指南構(gòu)建知識(shí)庫提供了一套通用的構(gòu)建方法。
[Abstract]:The purpose of this study is to model the clinical guide of static text form in the form of a suitable guide expression model to form a computer understandable form, combining ontology and knowledge base technology, transforming the clinical guide of the text form into a convenient reasoning knowledge base, which helps to improve the efficiency of the clinical guide and reduce the clinical practice. On the basis of this, it can also be used as an important module of the clinical decision support system of hypertension and to reduce the difficulty of design and construction. Research content 1. comparison and analysis of the related theories of medical knowledge base construction are summarized and analyzed through the related techniques of constructing the medical knowledge base, and the design of the knowledge base suitable for hypertension is found and the design of the hypertension medical knowledge base is found. The knowledge expression model improves the flexibility and scientificalness of the knowledge expression in the clinical guide.3., constructs the diagnostic method in the clinical guide of hypertension by using the Semantic Web Rule Language SWRL to construct the hypertension SWRL diagnosis rule base, providing the reasoning for the reasoning of the hypertension clinical guide ontology by using Jess as a reasoning machine and combining the clinical guide of hypertension with the hypertension clinical guide. The ontology and the SWRL diagnosis rule base realized the diagnostic reasoning of the hypertension knowledge base, and verified the accuracy and scientificity of the diagnostic reasoning with a set of test cases. This paper summarized the obstacles in clinical application of clinical guide in this paper, and put forward a new clinical guide digital method to solve the development of clinical guide. Hypertension is a common disease that threatens human health. Combining the clinical guidelines of hypertension with the ontology study to construct the knowledge base of the clinical guidelines for hypertension can not only improve the utilization of the guide in clinical practice, but also be used directly by the clinical decision support system. At the same time, the construction of the knowledge base of hypertension clinical guide is constructed. The method is also applicable to the construction of knowledge base for clinical guidelines of other diseases, and provides a general method for constructing clinical knowledge base.
