【摘要】：正患者男,52歲,因"抬重物后出現(xiàn)間斷腹部隱痛伴腰痛1周"入院,疼痛與體位、進食無明顯關系,對癥治療無好轉入院。查體:血壓166 mmHg/115 mmHg,腹軟,無壓痛;四肢血管搏動良好。主動脈CTA:腹腔干起始段管腔稍擴張,呈"雙腔征"改變,內膜破裂口位于近端,長約3mm(圖1A)。VR示腹腔干于胃左動脈開口10mm以遠閉塞,肝總動脈、脾動脈近端閉塞,可見2條側支血管供應胃十二指腸動脈及肝固有動脈,1支源
[Abstract]:The patient was 52 years old. He was admitted to hospital because of "intermittent abdominal pain with low back pain for 1 week after lifting heavy objects". There was no obvious relationship between pain and posture, eating, and no improvement in symptomatic treatment.Body examination: blood pressure 166 mmHg/115 Hg, soft abdomen, no tenderness, good blood vessel pulsation.Aortic CTAA: the initial segment of the celiac trunk was slightly dilated with a "double lumen sign". The rupture of the intima was located at the proximal end and was about 3 mm long. (1A).VR showed that the celiac trunk was distally occluded at the opening of the left gastric artery, the common hepatic artery and the proximal end of the splenic artery were occluded.It can be seen that two collateral vessels supply one branch of gastroduodenal artery and hepatic proper artery.
