慢性穩(wěn)定型心絞痛患者PCI術(shù)后循環(huán)內(nèi)皮祖細(xì)胞計(jì)數(shù)變化及其影響因素

【摘要】:目的研究慢性穩(wěn)定型心絞痛患者經(jīng)皮冠狀動(dòng)脈介入治療(PCI)前后循環(huán)內(nèi)皮祖細(xì)胞(EPC)的數(shù)量變化,并探討PCI術(shù)后CD34~+/KDR~+最大動(dòng)員幅度與患者內(nèi)皮損傷定量參數(shù)等臨床資料的相關(guān)性。方法選取93例診斷為慢性穩(wěn)定型心絞痛的患者,將其中成功行PCI術(shù)治療的72例患者作為PCI組,21例僅行冠狀動(dòng)脈造影術(shù)(CAG)保守治療的患者作為對(duì)照組。采集患者股動(dòng)脈鞘管或肘正中靜脈血樣,通過(guò)流式細(xì)胞儀對(duì)PCI組和對(duì)照組患者CAG術(shù)后即刻(基線水平)及PCI術(shù)后或CAG后1 h、3 h、5 h、7 h、24 h的EPC數(shù)量進(jìn)行測(cè)定。結(jié)果不同類(lèi)型的EPC在不同時(shí)間點(diǎn)及在PCI組和對(duì)照組兩組間的總體均數(shù)存在統(tǒng)計(jì)學(xué)差異(P0.05)。PCI組中,與基線水平相比,CD34~+/KDR~+在24 h,CD133-/CD34~+/KDR~+在3 h、5 h、24 h,CD133~+/CD34~+/KDR~+在5 h、7 h,CD133~+/KDR~+在3 h、5 h,CD133~+/CD34~+在24 h差異有統(tǒng)計(jì)學(xué)意義(P0.05)。對(duì)照組中,各類(lèi)型EPC在不同時(shí)間點(diǎn)計(jì)數(shù)與基線水平相比,無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。CD34~+/KDR~+最大動(dòng)員幅度與C反應(yīng)蛋白呈現(xiàn)負(fù)相關(guān),與體質(zhì)指數(shù)、服用血管緊張素轉(zhuǎn)化酶抑制劑或血管緊張素Ⅱ受體拮抗劑類(lèi)藥物的情況、血管內(nèi)皮損傷定量參數(shù)呈現(xiàn)正相關(guān)(P0.01)。結(jié)論P(yáng)CI導(dǎo)致的血管內(nèi)皮損傷可以引發(fā)EPC時(shí)間依賴(lài)性的動(dòng)態(tài)變化,同時(shí)可以促進(jìn)CD34~+/KDR~+的大幅動(dòng)員、遷移、歸巢,有利于受損血管內(nèi)皮修復(fù)愈合,減少并發(fā)癥,改善患者預(yù)后。
[Abstract]:Objective to study the quantitative changes of circulating endothelial progenitor cells (EPC) in patients with chronic stable angina pectoris before and after percutaneous coronary intervention (PCI), and to explore the correlation between the maximum mobilization range of CD34 ~ / KDR~ after PCI and the quantitative parameters of endothelial injury.Methods Ninety-three patients with chronic stable angina pectoris were selected and 72 patients who were successfully treated with PCI were selected as the control group. 21 patients in the PCI group were treated conservatively by coronary angiography.The blood samples of the femoral artery sheath tube or the median elbow vein were collected, and the number of EPC was measured immediately after CAG in PCI group and control group (baseline level) and 24 h after PCI or 1 h after PCI or 5 h and 7 h after CAG by flow cytometry.Results there were significant differences in the total mean of different types of EPC at different time points and between the PCI group and the control group.涓庡熀綰挎按騫崇浉姣,
