【摘要】：目的探討老年心力衰竭合并細菌性肺炎患者血清氨基末端腦鈉肽原(NT-pro BNP)、熱休克蛋白(HSP)70和降鈣素原(PCT)水平的變化及臨床意義。方法分別選取老年心力衰竭患者(心力衰竭組)、老年心力衰竭合并細菌性肺炎患者(合并肺炎組)、老年健康志愿者(對照組)各100例。檢測并對比各組血清NT-pro BNP、HSP70、PCT水平。所有患者均實施對癥支持治療,對比心力衰竭合并肺炎患者中不同心功能分級、死亡者和存活者血清NT-pro BNP、HSP70、PCT水平差異。結(jié)果合并肺炎組患者血清NT-pro BNP、HSP70、PCT水平均遠高于心力衰竭組和對照組(P0.05),且心力衰竭組血清NT-pro BNP、HSP70、PCT水平均遠高于對照組(P0.05);合并肺炎患者血清NT-pro BNP、HSP70、PCT水平均隨心功能分級的增加而明顯升高(P0.05),且死亡者血清NT-pro BNP、HSP70、PCT水平遠高于存活者(P0.05)。結(jié)論在老年心力衰竭合并細菌性肺炎患者中血清NT-pro BNP、HSP70、PCT水平均異常升高,且可用來評估患者的心功能及預(yù)后。
[Abstract]:Objective to investigate the changes and clinical significance of serum amino-terminal brain natriuretic peptide (NT-pro BNPN), heat shock protein (HSPN) 70 and procalcitonin (PCT) in elderly patients with heart failure complicated with bacterial pneumonia. The serum levels of NT-pro BNPN HSP70 and PCT were measured and compared in 100 elderly patients with heart failure complicated with bacterial pneumonia, 100 elderly healthy volunteers (control group) and 100 elderly patients with heart failure complicated with bacterial pneumonia. All patients were treated with symptomatic support therapy. To compare the different cardiac function grades in patients with heart failure and pneumonia. Results the serum levels of NT-pro BNPP HSP70 and PCT in the patients with pneumonia were significantly higher than those in the heart failure group and the control group (P 0.05), and the serum NT-pro BNPN HSP70 NT-pro levels in the heart failure group were significantly higher than those in the control group (P 0.05), and in the patients with pneumonia the levels were significantly higher than those in the control group (P 0.05). The serum levels of NT-pro BNPG HSP70 + PCT were significantly increased with the increase of cardiac function grade, and the serum NT-pro BNPG HSP70 / PCT levels were significantly higher in the dead patients than in the survivors. Conclusion the serum NT-pro BNPN HSP70PCT levels in the elderly patients with heart failure and bacterial pneumonia are all increased abnormally. It can be used to evaluate cardiac function and prognosis of patients.
