【摘要】：目的:應(yīng)用心臟磁共振心肌首過(guò)灌注技術(shù)(CMR-MPI)對(duì)急性病毒性心肌炎(AVM)進(jìn)行定量分析,并獲得灌注曲線下面積比與肌鈣蛋白(cTn)T的關(guān)系。方法:納入31例AVM患者(AVM組)與20例健康志愿者(對(duì)照組)。兩組行3.0T心臟磁共振掃描,測(cè)定CMR-MPI灌注參數(shù)(曲線下面積比、達(dá)峰時(shí)間及相對(duì)峰值信號(hào))和cTnT濃度。采用Spearman等級(jí)相關(guān)對(duì)CMR-MPI曲線下面積和血清cTnT濃度進(jìn)行統(tǒng)計(jì)分析。結(jié)果:曲線下面積比:灌注正常區(qū)、減低區(qū)及缺損區(qū)分別為(52.80±3.05)%、(37.64±4.87)%、(26.23±2.29)%,P0.05;相對(duì)峰值信號(hào)強(qiáng)度分別為(44.91±3.16)%、(33.96±4.98)%、(29.94±3.75)%,P0.05;達(dá)峰時(shí)間分別為(34.22±4.89)s、(37.85±7.51)s、(38.24±6.06)s,P0.05。AVM組與對(duì)照組血清cTnT濃度分別為(0.578±1.127)ng/ml、(0.011±0.006)ng/ml,P0.05。Spearman等級(jí)相關(guān)分析AVM組CMR-MPI曲線下面積比與血清cTnT濃度的關(guān)系,rs=-0.557,P0.05。結(jié)論:CMR-MPI可定量評(píng)估AVM心肌缺血程度,AVM患者血清cTnT濃度與CMR-MPI曲線下面積比呈負(fù)相關(guān),提示AVM心肌微循環(huán)障礙和心肌缺血可能是造成或加重心肌細(xì)胞損傷及cTnT釋放的原因之一。
[Abstract]:Objective: to study the quantitative analysis of acute viral myocarditis (AVM) by cardiac magnetic resonance myocardial first-pass perfusion (CMR-MPI). The relationship between area ratio under perfusion curve and cardiac troponin C TnTntr T was obtained. Methods: 31 patients with AVM and 20 healthy volunteers (control group) and 20 healthy volunteers (control group) underwent 3. 0T cardiac magnetic resonance scan to measure the perfusion parameters of CMR-MPI (area ratio under the curve). Peak time and relative peak signal) and cTnT concentration. The area under CMR-MPI curve and serum cTnT concentration were statistically analyzed by Spearman grade correlation. The relationship between the area under the CMR-MPI curve and the serum cTnT concentration in the AVM group was sr-0.755 P0.055.The results showed that the correlation between the area area and the serum cTnT concentration in the VM group was 0.578 鹵1.127ngml / ml 0.011 鹵0.006ngml / ml 0.011 鹵0.006ngml / ml 0.011 鹵0.006ngml / ml 0.011 鹵0.006ngml / ml respectively. Conclusion the serum cTnT concentration and area ratio under the CMR-MPI curve can be negatively correlated with the serum cTnT concentration in AVM patients with myocardial ischemia. The results suggest that myocardial microcirculation disturbance and myocardial ischemia in AVM may be one of the causes of myocardial cell injury and cTnT release.
