【摘要】：目的探討多排螺旋CT血管造影(CTA)成像對(duì)老年先天性椎動(dòng)脈變異的診斷價(jià)值。方法疑似先天性椎動(dòng)脈變異老年患者150例均通過(guò)數(shù)字減影血管造影(DSA)診斷,以DSA診斷結(jié)果作為"金標(biāo)準(zhǔn)",研究先天性椎動(dòng)脈變異病患實(shí)施多排螺旋CTA成像技術(shù)檢測(cè)的情況,對(duì)比診斷符合率,并針對(duì)CTA影像特征進(jìn)行觀察。結(jié)果確診131例椎動(dòng)脈變異,其中63例單側(cè)椎動(dòng)脈先天性狹窄,47例單干椎動(dòng)脈形成基底動(dòng)脈,21例椎動(dòng)脈行徑變異。多排螺旋CTA診斷老年先天性椎動(dòng)脈變異特異性89.47%(17/19),診斷敏感度為97.71%(128/131),準(zhǔn)確性96.67%(145/150),陽(yáng)性預(yù)測(cè)值達(dá)到98.46%(128/130),陰性預(yù)測(cè)值為85.00%(17/20),同椎動(dòng)脈變異DSA診斷結(jié)果相比無(wú)統(tǒng)計(jì)學(xué)差異,利用Kappa一致性檢驗(yàn)具備一致性。結(jié)論多排螺旋CTA技術(shù)診斷老年先天性椎動(dòng)脈變異準(zhǔn)確性良好,能夠?qū)ψ祫?dòng)脈變異形態(tài)、特征做直觀判斷,減少漏診或誤診風(fēng)險(xiǎn)。
[Abstract]:Objective to evaluate the diagnostic value of multi-slice spiral CT angiography (CTAA) in the diagnosis of congenital vertebral artery variation in the elderly. Methods 150 patients with suspected congenital vertebral artery variation were diagnosed by digital subtraction angiography (DSAs). The diagnostic results of DSA were used as the "golden standard" to study the detection of multiple spiral CTA imaging in patients with congenital vertebral artery variation, the diagnostic coincidence rate was compared, and the characteristics of CTA images were observed. Among 63 cases of congenital stenosis of unilateral vertebral artery, 47 cases were single stem vertebral artery and 21 cases were vertebral artery deviation. The specificity of multislice spiral CTA in diagnosis of congenital vertebral artery variation in the elderly was 89.47 / 19. The diagnostic sensitivity was 97.71 / 128 / 131, and the accuracy was 96.6775 / 150, Yang. The predictive value of sex reached 98.460.128 / 130%, and the negative predictive value was 85.00 / 20 / 20. There was no significant difference between the results of DSA diagnosis of vertebral artery variation. Conclusion Multi-row helical CTA technique has good accuracy in diagnosing congenital vertebral artery variation in the elderly, which can directly judge the shape and characteristics of vertebral artery variation and reduce the risk of missed diagnosis or misdiagnosis.
