【摘要】：目的研究老年患者高敏肌鈣蛋白(hs-cTn)T升高程度在慢性心力衰竭(CHF)與急性心肌梗死(AMI)鑒別診斷中的價(jià)值。方法372例老年CHF患者作為CHF組296例老年AMI患者作為AMI組,并以238例健康老年體檢人群為對(duì)照組,比較三組hs-cTnT水平。受試者工作特征(ROC)曲線分析hs-cTnT對(duì)CHF的診斷切點(diǎn)。結(jié)果 hs-cTnT在AMI組(3.245±1.356)ng/ml、CHF組(0.298±0.124)ng/ml及對(duì)照組(0.009±0.003)ng/ml的血漿濃度依次顯著降低(P0.05);ROC曲線評(píng)價(jià)hs-cTnT對(duì)CHF的診斷價(jià)值,曲線下面積(AUC)為0.746(95%CI:0.696~0.864).診斷范圍為0.09~2.38ng/ml;對(duì)AMI的診斷價(jià)值,AUC為0.865(95%CI:0.715~0.965),診斷切點(diǎn)為2.38ng/ml;不同美國紐約心臟病學(xué)會(huì)(NYHA)心功能CHF患者及對(duì)照組hs-cTnT比較,Ⅳ級(jí)(0.369±0.117)ng/ml、Ⅲ級(jí)(0.267±0.102)ng/ml、Ⅱ級(jí)(0.142±0.084)ng/ml及對(duì)照組(0.009±0.003)ng/ml血漿濃度依次降低,hs-cTnT與NYHA心功能經(jīng)Pearson等級(jí)相關(guān)分析呈正相關(guān)(r=0.825,P0.05)。結(jié)論hs-cTnT作為心肌損傷特異性標(biāo)志物在CHF及AMI的早期鑒別及CHF病情評(píng)估有重要應(yīng)用價(jià)值。
[Abstract]:Objective to study the value of the elevation of troponin cTnTnTnTc in the elderly patients with chronic heart failure (CHF) and acute myocardial infarction (AMI). Methods 372 elderly patients with CHF were treated as CHF group, 296 elderly AMI patients as AMI group. 238 healthy elderly people were taken as control group. The diagnostic point of hs-cTnT for CHF was analyzed by comparing the three groups of hs-cTnT levels. Results the plasma concentrations of hs-cTnT in AMI group (3.245 鹵1.356g / ml) and control group (0.298 鹵0.124ng / ml) and control group (0.009 鹵0.003ng / ml) were significantly lower than those in control group (P 0.05ng / ml) to evaluate the value of hs-cTnT in the diagnosis of CHF.
