【摘要】：目的探討冠狀動(dòng)脈CTA時(shí)不同對(duì)比劑注射方案對(duì)圖像質(zhì)量的影響,優(yōu)化注射方案。方法選取體重指數(shù)相似患者320例,分為A、B、C、D四組,進(jìn)行冠狀動(dòng)脈CTA。A組對(duì)比劑采用雙相注射方案,先期注射對(duì)比劑70 ml,而后注射生理鹽水20 ml;B組采用雙相注射方案,先期注射對(duì)比劑50 ml,而后注射混合液(生理鹽水-對(duì)比劑1∶1)20 ml,C組采用三相注射方案,先期注射對(duì)比劑50 ml,第二時(shí)相注射混合液(生理鹽水-對(duì)比劑1∶1)20 ml,而后無(wú)縫隙注射生理鹽水20 ml;D組采用雙相注射方案,先期注射對(duì)比劑50 ml,而后注射混合液(生理鹽水-對(duì)比劑1∶1)20 ml,掃描時(shí)間較常規(guī)延遲5 s。比較四組冠狀動(dòng)脈CTA圖像中冠狀動(dòng)脈三大分支和升主動(dòng)脈(AO)CT值之間有無(wú)差異,比較四組上腔靜脈、右心室硬化偽影發(fā)生率及室間隔顯示率。結(jié)果與A組比較,B、C兩組對(duì)比劑雖減少10 ml,但三組間AO及冠狀動(dòng)脈三大分支CT值比較無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),而D組CT值較A、B、C三組降低(P0.05);B組上腔靜脈、右心室硬化偽影發(fā)生率高于A、C、D三組(P0.05),C、D兩組室間隔顯示率無(wú)統(tǒng)計(jì)學(xué)差異(P0.05),但高于A、B兩組(P0.05)。結(jié)論采用三時(shí)相對(duì)比劑注射方案,可以有效降低對(duì)比劑用量,保證圖像質(zhì)量,清晰顯示室間隔,為冠狀動(dòng)脈疾病的診斷提供影像依據(jù)。
[Abstract]:Objective to investigate the effect of different contrast media injection on image quality in coronary artery CTA and optimize the injection protocol. Methods 320 patients with similar body mass index were divided into four groups. The coronary artery CTA.A group was treated with biphasic injection. Group B was treated with biphasic injection, group B with 20 ml saline, and group B with 50 ml of contrast agent, and then with a mixture of saline and contrast agent (1: 1 1: 20 ml / C). First, 50 ml contrast agent was injected, the second phase was injected with mixed solution (1: 1 1: 20 ml of saline), and then the group D was given biphasic injection of 20 ml saline without slit injection. 50 ml contrast agent was injected first, and then mixed solution (1: 1ml contrast agent) was injected. The scanning time was delayed by 5 s. The differences between the three branches of coronary artery and ascending aorta in CTA images of four groups were compared. The superior vena cava was compared among the four groups. Results compared with group A, the incidence of right ventricular sclerosis artifact and the rate of ventricular septal display were decreased by 10 ml, but the CT values of AO and coronary artery branches in group D were not significantly higher than those in group A (P 0.05), but the CT value in group D was higher than that in group A (P < 0.05). The superior vena cava was decreased in group B. The incidence of right ventricular sclerosis artifact was higher than that of the three groups (P 0.05 and P 0.05), but it was higher than that of group A B (P 0.05). Conclusion the dosage of contrast agent can be reduced effectively and the image quality can be ensured by using the three-time contrast agent injection regimen, and the results are as follows: (1) the incidence of right ventricular sclerosis artifact is higher than that of the control group (P 0.05), but it is higher than that of group A B (P 0.05). The clear display of ventricular septum provides imaging basis for the diagnosis of coronary artery disease.
