【摘要】：目的:探討強化劑量阿托伐他汀對不穩(wěn)定型心絞痛患者經(jīng)皮冠脈介入術(PCI)圍手術期B7-H4的表達的影響。方法:選取2012年10月至2014年10月廣西醫(yī)科大學第一附屬醫(yī)院和廣西中醫(yī)藥大學第一附屬醫(yī)院心內(nèi)科收治的不穩(wěn)定型心絞痛患者80例,隨機分為常規(guī)劑量阿托伐他汀組和強化劑量阿托伐他汀組,每組40例,兩組患者分別于PCI術前及術后18~24 h采集外周靜脈血,采用酶聯(lián)免疫吸附法(ELISA)檢測血清可溶性B7-H4(s B7-H4)、白介素(IL)-4、IL-10、γ干擾素(IFN-γ)水平,熒光定量PCR(FQ-PCR)法檢測外周血單核細胞(PBMC)B7-H4 m RNA相對表達量。結(jié)果:兩組患者PCI術后的IL-10、s B7-H4水平和B7-H4 m RNA表達量均較術前升高,其中強化劑量組升高更為顯著(P0.05)。兩組患者PCI術后IL-4、IFN-γ水平均較術前降低,且強化組較常規(guī)組下降更為明顯(P0.05)。結(jié)論:強化劑量阿托伐他汀可能通過促進B7-H4表達,調(diào)節(jié)炎性因子IL-4、IFN-γ水平,從而抑制不穩(wěn)定型心絞痛患者PCI術后免疫炎癥。
[Abstract]:Objective: to investigate the effect of intensive dose Atto vastatin on the expression of B7-H4 in patients with unstable angina pectoris undergoing percutaneous coronary intervention (PCI). Methods: from October 2012 to October 2014, the first affiliated Hospital of Guangxi Medical University was selected. And 80 patients with unstable angina pectoris admitted to the Department of Cardiology, the first affiliated Hospital of Guangxi University of traditional Chinese Medicine, They were randomly divided into routine dose Atto vastatin group and enhanced dose Atto vastatin group with 40 patients in each group. Peripheral venous blood was collected before and after PCI for 1824 hours. Elisa was used to detect the levels of serum soluble B7-H _ 4s B _ 7-H _ 4N, IL-10 and IFN- 緯. The relative expression of B7-H4 m RNA in peripheral blood monocytes was detected by fluorescence quantitative PCR- FQ-PCR.Results: the levels of IL-10s B7-H4 and the expression of B7-H4m RNA in peripheral blood monocytes were increased after PCI in both groups. The levels of IL-4 and IFN- 緯 after PCI in both groups were lower than those in the preoperative group, and the levels of IL-4 and IFN- 緯 in the enhanced group were significantly lower than those in the normal group. Conclusion: the enhanced dose of Atto vastatin may promote the expression of B7-H4. To regulate the level of IL-4 and IFN- 緯 in patients with unstable angina pectoris and suppress the immune inflammation after PCI.
