【摘要】：目的:探討參芪地黃湯加減聯(lián)合西藥治療高血壓腎病的療效。方法:根據(jù)隨機數(shù)字表法將2012年1月—2016年7月本院收治的180例高血壓腎病患者分為對照組(90例)與觀察組(90例),對照組患者接受硝苯地平聯(lián)合卡托普利治療,觀察組患者在硝苯地平、卡托普利基礎(chǔ)上再聯(lián)合使用參芪地黃湯,現(xiàn)對比分析兩組患者的治療效果。結(jié)果:(1)組內(nèi)不同時間點之間的血壓比較:治療前、治療1個月后、治療3個月后,觀察組與對照組3個時間點之間的舒張壓(DBP)、收縮壓(SBP)比較差異均有統(tǒng)計學(xué)意義(P0.05),治療前的DBP、SBP均顯著高于治療1個月后、治療3個月后(P0.05),治療1個月后的DBP、SBP均顯著高于治療3個月后(P0.05)。(2)組內(nèi)不同時間點之間的腎功能指標比較:治療前、治療1個月后、治療3個月后,觀察組與對照組3個時間點之間的血清胱抑素C(SCys C)、估算腎小球濾過率(eGFR)、尿微量白蛋白(Um Alb)相比差異均有統(tǒng)計學(xué)意義(P0.05);治療前的SCys C、Um Alb顯著高于治療1個月后、治療3個月后(P0.05),eGFR顯著低于治療1個月后、治療3個月后(P0.05);治療1個月后的SCys C、Um Alb顯著高于治療3個月后(P0.05),eGFR顯著低于治療3個月后(P0.05)。不同時間點兩組之間腎功能指標比較:治療前,兩組患者SCys C、eGFR、Um Alb相比差異均無統(tǒng)計學(xué)意義(P0.05);治療1個月后、治療3個月后,觀察組SCys C、Um Alb顯著低于對照組,eGFR均顯著高于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:參芪地黃湯加減聯(lián)合西藥治療高血壓腎病的療效確切,在控制血壓的同時有效保護患者腎功能,值得臨床推廣應(yīng)用。
[Abstract]:Objective: to investigate the efficacy of Shenqi Dihuang decoction combined with western medicine in the treatment of hypertensive nephropathy. Methods: according to random number table, 180 patients with hypertension nephropathy admitted in our hospital from January 2012 to July 2016 were divided into control group (n = 90) and control group (n = 90). 90 patients in the observation group were treated with nifedipine combined with captopril, while the patients in the control group were treated with nifedipine and captopril. The patients in the observation group were treated with Shenqi Dihuang decoction on the basis of nifedipine and captopril. After 3 months of treatment, there were significant differences in DBP, SBP and SBP between the observation group and the control group at three time points. The SBP before treatment was significantly higher than that after 1 month treatment. After 3 months of treatment and 1 month after treatment, the levels of SBP in DBP group were significantly higher than those in the control group at different time points: before treatment, 1 month after treatment, and 3 months after treatment, there were significant differences in renal function index between the two groups after 3 months of treatment, 1 month after treatment, 3 months after treatment, 1 month after treatment, 3 months after treatment, 3 months after treatment, 3 months after treatment, 3 months after treatment. Compared with the control group, the serum cystatin cystatin (Cnscys), estimated glomerular filtration rate (GFR) and urinary microalbumin (Um Alb) in the observation group were significantly higher than those in the control group (P 0.05), and the SCys Con Um Alb before treatment was significantly higher than that after 1 month treatment. After 3 months of treatment, P0.05 eGFR was significantly lower than that after 1 month treatment. After 3 months of treatment, the levels of SCys Con Um Alb were significantly higher than those after 3 months of treatment. The renal function indexes of the two groups at different time points were significantly lower than those after 3 months of treatment. There was no significant difference in SCys CfR Um Alb between the two groups, but after one month of treatment and 3 months of treatment, the SCys CU Um Alb in the observation group was significantly lower than that in the control group, which was significantly higher than that in the control group. Conclusion: Shenqi Dihuang decoction combined with western medicine is effective in the treatment of hypertensive nephropathy, which can effectively protect renal function while controlling blood pressure, and is worth popularizing in clinic.
