肝囊腫和血管瘤的DWI診斷及鑒別診斷價(jià)值

目的:研究磁共振擴(kuò)散加權(quán)成像對肝囊腫和肝血管瘤的診斷和鑒別診斷價(jià)值。 方法:收集80例病人共140個病變(囊腫80個,血管瘤60個),所有患者均經(jīng)臨床和/或綜合影像學(xué)檢查證實(shí)綜合判斷已確診的病例,對80例病人均進(jìn)行常規(guī)MR掃描和磁共振擴(kuò)散加權(quán)成像掃描,分析肝囊腫和肝血管瘤DWI的特點(diǎn)并測量不同b值下肝臟實(shí)質(zhì)和病變的表觀彌散系數(shù)ADC值、肝實(shí)質(zhì)信噪比SNR、病灶-肝臟對比噪聲比CNR及其相關(guān)關(guān)系。 結(jié)果:DWI圖像顯示肝囊腫病灶(98.5%)呈低信號或略高信號,肝血管瘤病灶(96.7%)呈均勻高信號,b值與肝臟SNR、肝囊腫和血管瘤病灶與肝實(shí)質(zhì)CNR、肝囊腫和血管瘤ADC值均存在顯著負(fù)相關(guān)。肝囊腫和血管瘤與肝臟的ADC值均存在顯著的差異(P<0.05),隨著b值的增大,肝囊腫和血管瘤ADC值、二者與肝實(shí)質(zhì)的CNR均降低,尤以囊腫明顯,當(dāng)b>500s/mm2時(shí)囊腫的信號與肝臟幾乎一致。當(dāng)b值選用500s/mm2時(shí),肝囊腫和血管瘤的ADC值不同,分別為(3.24±1.27)x10-3mm2/s和(2.53±0.53)x10-3mm2/s。 結(jié)論:磁共振擴(kuò)散加權(quán)成像有助于對肝...
