阿德福韋酯治療前后乙型肝炎病毒Pre-S區(qū)基因突變分析

乙型肝炎病毒(hepatitis B virus,HBV)感染是全球性的重要公共衛(wèi)生問題,嚴(yán)重影響人類健康。HBV逆轉(zhuǎn)錄酶缺乏校正功能,在選擇壓力作用下,HBV在復(fù)制過程中會產(chǎn)生大量變異株。Pre-S區(qū)是HBVDNA中變異最大的區(qū)段,容易出現(xiàn)缺失突變。Pre-S區(qū)缺失熱點(diǎn)區(qū)域?yàn)镻re-S1的3’末端和Pre-S2的5’末端,可伴有Pre-S2起始密碼子的缺失或點(diǎn)突變,而且在B和C基因型中發(fā)生頻率高。大量研究證實(shí),Pre-S區(qū)缺失突變可作為一個獨(dú)立危險因素,與肝細(xì)胞癌發(fā)生具有密切關(guān)系。因此有效抑制HBV DNA尤其是Pre-S區(qū)缺失突變毒株的復(fù)制可能有利于減少HCC的發(fā)生。核苷(酸)類似物是臨床上抗HBV的常用藥物,能夠抑制HBV DNA復(fù)制,降低血清ALT水平,改善肝組織學(xué)。但是有研究表明,Pre-S區(qū)缺失突變也可在使用核苷(酸)類似物治療是出現(xiàn),而且Pre-S區(qū)缺失突變毒株對于這類藥物的反應(yīng)性并不清楚。因此,解決這一疑問是關(guān)系到臨床用藥的安全性問題。目的:觀測阿德福韋酯(ADV)治療慢性HBV患者基線和48W時血清中HBV DNA Pre-S區(qū)突變情況。方法:提取61位患者基線和48...
