急性胰腺炎患者腹腔感染的危險(xiǎn)因素和病原學(xué)分析

【摘要】:目的探討急性胰腺炎患者腹腔感染的危險(xiǎn)因素和病原學(xué)特征,為臨床防治腹腔感染提供依據(jù)。方法回顧性分析醫(yī)院2011年1月-2016年6月收治急性胰腺炎患者187例,分析不同臨床特征的急性胰腺炎患者腹腔感染風(fēng)險(xiǎn)及危險(xiǎn)因素和主要病原菌。結(jié)果不同病因的急性胰腺炎患者腹腔感染風(fēng)險(xiǎn)不同(P0.05);入院時(shí)SOFA評(píng)分≥2分、APACHEⅡ評(píng)分≥8分、Ranson評(píng)分≥3分、CTSI評(píng)分≥4分、臟器功能損傷和休克的急性胰腺炎患者腹腔感染率顯著升高(P0.05);多因素回歸分析顯示,Ranson評(píng)分≥3分、CTSI評(píng)分≥4分、休克和臟器功能損傷是急性胰腺炎患者腹腔感染的危險(xiǎn)因素(P0.05);18例腹腔感染的患者共培養(yǎng)出病原菌32株,其中革蘭陰性菌18株占56.25%,革蘭陽(yáng)性菌14株占43.75%,最常見(jiàn)的革蘭陰性菌是銅綠假單胞菌,最常見(jiàn)的革蘭陽(yáng)性菌是金黃色葡萄球菌。結(jié)論 Ranson評(píng)分≥3分、CTSI評(píng)分≥4分、休克和臟器功能損傷是急性胰腺炎患者腹腔感染的危險(xiǎn)因素,感染以革蘭陰性菌為主。
