【摘要】：目的:比較食管胃靜脈曲張精準(zhǔn)內(nèi)鏡治療,在賁門上方治療組、賁門下方治療組、賁門上方和下方聯(lián)合治療組3組的療效,觀察血管消失及不良作用.方法:食管胃靜脈曲張破裂出血患者104例,精準(zhǔn)內(nèi)鏡治療分為賁門上方治療組68例,賁門下方治療組15例,賁門上方和下方聯(lián)合治療組21例.比較3組靜脈曲張治療的總有效率、有效率、顯效率、再出血率、并發(fā)癥發(fā)生率.結(jié)果:治療后3組靜脈曲張總改善率、有效率、顯效率,賁門上方組為63.2%、23.5%和39.7%,賁門下方組為80.0%、20.0%和60.0%,賁門上和下方聯(lián)合治療組90.5%、28.6%和61.9%.3組患者治療后食管胃靜脈曲張總有效率有統(tǒng)計學(xué)差異(?2=6.444,P=0.039),有效率、顯效率有統(tǒng)計學(xué)差異(?2=6.235,P=0.044).治療后2 wk和3 mo內(nèi)再出血率,賁門上方治療組為11.8%和19.1%,賁門下方治療組為6.7%和13.3%,賁門上方和下方聯(lián)合治療組為9.5%和14.3%.3組再出血率均很低,但無統(tǒng)計學(xué)差異(P0.05).3組均無嚴重并發(fā)癥發(fā)生,3組間并發(fā)癥的發(fā)生率分別為30.9%、33.3%和38.1%,沒有統(tǒng)計學(xué)差異(P0.05).結(jié)論:食管胃靜脈曲張精準(zhǔn)內(nèi)鏡治療選擇賁門上方和下方聯(lián)合治療療效好.
[Abstract]:Objective: to compare the efficacy of accurate endoscopic treatment of esophageal and gastric varices in the treatment group above the cardia, the treatment group below the cardia and the combined treatment group above and below the cardia, and to observe the disappearance and adverse effects of blood vessels. Methods: 104 patients with esophageal and gastric variceal bleeding were divided into three groups: superior cardia treatment group (n = 68), inferior cardia treatment group (n = 15) and combined treatment group (n = 21). The total effective rate, rebleeding rate and complications rate of varicose veins in the three groups were compared.
