【摘要】：目的探討非酒精性脂肪肝(NAFLD)對新發(fā)糖尿病的影響。方法采用前瞻性隊列研究,以參加2006年7月-2007年10月開灤集團公司健康體檢的離退休職工為研究對象,排除既往有糖尿病病史、飲酒史以及資料不全者,最終44 019例納入隊列。對所有參與者進行問卷調(diào)查、體格檢查和實驗室檢查,每兩年隨訪一次。將研究對象按基線有無NAFLD分為NAFLD組和非NAFLD組,比較兩組新發(fā)糖尿病檢出率和累積發(fā)病率,采用多因素Cox回歸模型分析NAFLD對新發(fā)糖尿病的影響。結(jié)果在全人群中,NAFLD組新發(fā)糖尿病檢出率和累積發(fā)病率(分別為12.3%、12.5%)均高于非NAFLD組(分別為4.5%、4.7%),差異有統(tǒng)計學(xué)意義(P0.01)。多因素Cox回歸模型結(jié)果顯示,在校正年齡、性別、腰圍等因素后,NAFLD組發(fā)生糖尿病的風(fēng)險是非NAFLD組的1.61倍(95%CI 1.47~1.75)。結(jié)論基線NAFLD是新發(fā)糖尿病的獨立危險因素。
[Abstract]:Objective to investigate the effect of non-alcoholic fatty liver (NAFLD) on new diabetes mellitus. Methods A prospective cohort study was conducted among retired workers who participated in the physical examination of Kailuan Group Company from July 2006 to October 2007. The previous history of diabetes mellitus, drinking history and incomplete data were excluded. In the end, 44 019 cases were added to the queue. All participants were followed up every two years by questionnaire, physical examination and laboratory examination. The subjects were divided into NAFLD group and non-NAFLD group according to the presence or absence of NAFLD at baseline. the detection rate and cumulative incidence of new diabetes mellitus were compared between the two groups. The effect of NAFLD on new diabetes mellitus was analyzed by multivariate Cox regression model. Results the detection rate and cumulative incidence of new diabetes mellitus in NAFLD group (12.3%, 12.5%) were significantly higher than those in non-NAFLD group (4.5%, 4.7%) (P 0.01). The results of multivariate Cox regression model showed that after adjusting for age, sex, waist circumference and other factors, the risk of diabetes in NAFLD group was 1.61 times higher than that in non-NAFLD group (95%CI 1.47 鈮，
