木瓜三萜片與奧美拉唑膠囊聯(lián)用治療非甾體抗炎藥致上消化道出血臨床研究

【摘要】:目的:觀察木瓜三萜片與奧美拉唑膠囊聯(lián)用治療非甾體抗炎藥致上消化道出血的療效與安全性及其對胃黏膜攻擊因子(ICAM-1和PAF)和防御因子(COX-1、PGE2、EGF和TFF1)水平的影響。方法:將124例非甾體抗炎藥致上消化道出血患者完全隨機(jī)分成2組:對照組62例、治療組62例。對照組單用奧美拉唑治療,治療組采用木瓜三萜片聯(lián)合奧美拉唑治療,2組療程均為4周。觀察2組治療效果及安全性,治療前后取靜脈血采用ELISA試劑盒檢測血清中ICAM-1、PAF和COX-1、PGE2、EGF、TFF1水平的變化。結(jié)果:治療后,治療組癥狀積分(腹脹、腹痛、反酸、噯氣等)明顯低于對照組,總有效率(95.16%)明顯高于對照組(79.03%);治療前兩組血清中ICAM-1、PAF和COX-1、PGE2、EGF、TFF1水平?jīng)]有差異,分別用相應(yīng)的藥物治療后,對照組弱有改善,但是與木瓜三萜聯(lián)用后有顯著改善,未見明顯不良反應(yīng),與對照組比較具有顯著性差異。結(jié)論:木瓜三萜片與奧美拉唑膠囊聯(lián)用治療非甾體抗炎藥引起的上消化道出血療效更好,無明顯不良反應(yīng),值得臨床推廣應(yīng)用。
[Abstract]:Objective: to observe the efficacy and safety of papaya triterpenoid tablets combined with omeprazole capsules in the treatment of upper gastrointestinal bleeding induced by non-steroidal anti-inflammatory drugs (NSAIDs), and to evaluate the efficacy and safety of the combination of triterpene tablets and omeprazole capsules in the treatment of upper gastrointestinal The influence of EGF and TFF1 levels. Methods: 124 patients with upper gastrointestinal bleeding caused by NSAIDs were randomly divided into two groups: control group (n = 62) and treatment group (n = 62). There was no difference in serum levels of ICAM-1,PAF and COX-1,PGE2,EGF,TFF1 between the two groups before treatment. After treatment with the corresponding drugs, the control group was weakly improved, but there was significant improvement in combination with papaya triterpene, and no obvious adverse reactions were observed. Compared with the control group, there was significant difference. Conclusion: the combination of papaya triterpenoid tablet and omeprazole capsule is more effective in the treatment of upper gastrointestinal bleeding caused by non-steroidal anti-inflammatory drugs and has no obvious adverse reaction. It is worth popularizing and applying in clinical practice.
