自身免疫性肝病患者外周血Th17與Treg細胞及其相關因子表達的研究

【摘要】:目的研究自身免疫性肝。ˋILD)患者外周血中輔助性T細胞17(Th17)、調節(jié)性T細胞(Treg細胞)及其細胞因子白細胞介素-17(IL-17)、轉化生長因子-β(TGF-β)的表達情況,探討Th17與Treg細胞在AILD發(fā)病機制中的作用及其臨床意義。方法選擇46例AILD患者,其中28例自身免疫性肝炎患者(AIH組),18例原發(fā)性膽汁性肝硬化患者(PBC組)和20例健康人(對照組)做為研究對象,使用流式細胞術對AIH組、PBC組及對照組的外周血單個核細胞(PBMCs)中的Treg及Th17細胞的比例進行檢測;應用酶聯(lián)免疫吸附法(ELISA)檢測血清中Th17的相關因子IL-17和Treg的相關因子TGF-β含量。測得數(shù)據(jù)采用SPSS19.0統(tǒng)計軟件進行分析。結果AIH組和PBC組患者PBMCs中Th17細胞比例高于對照組(P<0.01),Treg細胞顯著低于對照組(P0.05);AIH組和PBC組患者IL-17的含量較對照組升高(P 0.05),,血清TGF-β的含量較對照組降低(P 0.01)。結論在AILD患者中,Th17、Treg細胞的變化可能在AILD發(fā)病機制中發(fā)揮重要作用,其變化可能是引起機體免疫系統(tǒng)平衡傾向促炎方向而導致AILD的發(fā)病的機制之一;對Th17/Treg的觀察可用于AILD疾病進展情況監(jiān)測及療效觀察;IL-17和TGF-β與AILD的相關性較強,二者結合可提高診斷AILD的敏感性。
[Abstract]:Objective to study the peripheral blood helper T cell 17 (Th17), regulatory T cell (Treg cell) and its cytokine interleukin-17 (IL-17) in patients with autoimmune liver disease (AILD). The expression of transforming growth factor- 尾 (TGF- 尾) and the role of Th17 and Treg cells in the pathogenesis of AILD and its clinical significance were investigated. Methods Forty-six patients with AILD, including 28 patients with autoimmune hepatitis (AIH group), 18 patients with primary biliary cirrhosis (PBC group) and 20 healthy people (control group), were selected as subjects. Flow cytometry was used to treat AIH group. The proportion of Treg and Th17 cells in (PBMCs) of peripheral blood mononuclear cells in PBC group and control group were detected. Enzyme linked immunosorbent assay (ELISA) was used to detect the levels of Th17 related factor IL-17 and Treg related factor TGF- 尾 in serum. The measured data were analyzed by SPSS19.0 software. Results the percentage of Th17 cells in PBMCs in AIH group and PBC group was higher than that in control group (P < 0. 01). The percentage of Th17 cells in PBMCs was significantly lower than that in control group (P < 0. 01). The content of IL-17 in AIH group and PBC group was higher than that in control group (P0. 05), and the content of serum TGF- 尾 was lower than that in control group (P0. 01). Conclusion in AILD patients, the changes of Th17,Treg cells may play an important role in the pathogenesis of AILD, which may be one of the mechanisms that lead to the pro-inflammatory direction of the immune system and lead to the pathogenesis of AILD. The observation of Th17/Treg can be used to monitor the disease progression and observe the curative effect of AILD, IL-17 and TGF- 尾 have strong correlation with AILD, and the combination of IL-17 and TGF- 尾 can improve the sensitivity of AILD diagnosis.
