【摘要】：研究目的潰瘍性結(jié)腸炎是一種病因、發(fā)病機制尚未明確的腸道慢性非特異性炎癥,具有反復(fù)發(fā)作、遷延不愈的特點。準確及時地判斷疾病的活動度及嚴重度對于調(diào)整治療方案、減輕患者經(jīng)濟負擔(dān)以及改善患者預(yù)后具有重要意義。本研究通過對潰瘍性結(jié)腸炎在不同病情分期、不同嚴重程度時血小板計數(shù)的差異進行統(tǒng)計比較與Meta分析,進一步了解血小板計數(shù)在潰瘍性結(jié)腸炎活動度與嚴重度評價方面的指標效應(yīng),明確血小板計數(shù)分析在潰瘍性結(jié)腸炎患者中的臨床意義,為臨床依據(jù)血小板計數(shù)判斷疾病活動度和嚴重度提供參考依據(jù)。研究方法通過檢索Pubmed及CNKI數(shù)據(jù)庫,對截止至2014年12月11日發(fā)表的有關(guān)血小板計數(shù)與潰瘍性結(jié)腸炎相關(guān)性的文獻進行篩選。根據(jù)納入和排除標準,提取符合條件的文獻中正常對照組、緩解期組及活動期組的例數(shù)及血小板均數(shù)值,并運用Stata軟件進行數(shù)據(jù)合并及分析。研究結(jié)果共檢索出英文文獻561篇,中文文獻335篇,通過篩選最終納入符合條件的18篇文獻。以血小板均數(shù)值作為評價指標,兩兩合并,在正常對照組和緩解期組:n=18,SMD=-0.30,95%CI(-0.50~-0.09);緩解期組和活動期組:n=16,SMD=-0.72,95%CI(-0.95~-0.48);輕度組和中度組:n=5,SMD=-0.81,95%CI(-1.08~-0.54);中度組和重度組:n=5,SMD=-0.85,95%CI(-1.36~-0.34);合并結(jié)果均具有統(tǒng)計學(xué)意義。研究結(jié)論①血小板計數(shù)在潰瘍性結(jié)腸炎患者中較正常人群升高。②血小板計數(shù)在潰瘍性結(jié)腸炎的活動期較緩解期升高,可以作為評價潰瘍性結(jié)腸炎活動度的指標之一。③血小板計數(shù)與潰瘍性結(jié)腸炎的嚴重度呈正相關(guān),可作為評價潰瘍性結(jié)腸炎嚴重度的指標之一。
[Abstract]:Objective ulcerative colitis is a kind of etiology. The pathogenesis of ulcerative colitis is not clear. It is important to judge the activity and severity of the disease accurately and timely for adjusting the treatment plan, reducing the economic burden of the patients and improving the prognosis of the patients. In this study, the differences of platelet count in different stages and severity of ulcerative colitis were compared and analyzed by Meta. To further understand the effect of platelet count on the evaluation of activity and severity of ulcerative colitis, and to clarify the clinical significance of platelet count analysis in patients with ulcerative colitis. To provide reference for clinical diagnosis of disease activity and severity based on platelet count. Methods by searching the databases of Pubmed and CNKI, the literatures about the correlation between platelet count and ulcerative colitis as of December 11, 2014 were selected. According to the inclusion and exclusion criteria, the cases and platelet values of normal control group, remission group and active group were extracted, and the data were combined and analyzed by Stata software. Results A total of 561 articles in English and 335 articles in Chinese were retrieved. The platelet mean value was used as the evaluation index. In the normal control group and the remission group, the normal control group and the remission stage group: nongy18SMD-0.30 ~ 95 CI (-0.50 ~ -0.09); In the remission stage and active stage group, the CI was 0.95% (-0.95U -0.48), and that in the mild and moderate groups was (-1.08U -0.54). In moderate group and severe group, there was significant difference in CI (-1.36 ~ -0.34) between SMD-0.85 and SMD-0.85, and the combined results were statistically significant. Conclusions\ The platelet count is positively correlated with the severity of ulcerative colitis and can be used as an index to evaluate the severity of ulcerative colitis.
