【摘要】：目的:為臨床合理用藥及避免藥物性肝損害提供參考。方法:利用醫(yī)院集中監(jiān)測(cè)方法,回顧性分析某院2012年7月-2013年6月所有住院病歷,發(fā)掘出藥物性肝損害病例121例,詳細(xì)記錄患者資料,歸納、總結(jié)出引發(fā)藥物性肝損害的藥品種類,并對(duì)其危險(xiǎn)因素進(jìn)行分析。結(jié)果:引發(fā)藥物性肝損害前3類藥物分別是抗菌藥物、抗腫瘤藥和中成藥;性別與年齡、過(guò)敏體質(zhì)、煙酒史、肝病史和免疫力低下是引起藥物性肝損害的危險(xiǎn)因素。臨床表現(xiàn)以肝細(xì)胞損害型最為常見,有64例(占52.89%)。結(jié)論:引發(fā)藥物性肝損害的藥品種類較多,應(yīng)嚴(yán)格把握用藥指征,盡量避免聯(lián)合用藥,對(duì)老年和存在危險(xiǎn)因素患者應(yīng)權(quán)衡利弊決定用藥方案。
[Abstract]:Objective: to provide reference for clinical rational use of drugs and prevention of drug-induced liver damage. Methods: by means of centralized monitoring in hospital, all the medical records of a hospital from July 2012 to June 2013 were retrospectively analyzed, 121 cases of drug-induced liver damage were found out, and the data of the patients were recorded and summarized in detail. The types of drugs causing drug-induced liver damage were summarized and the risk factors were analyzed. Results: the first three kinds of drugs causing drug-induced liver injury were antimicrobial drugs, antitumor drugs and Chinese patent drugs, sex and age, allergic constitution, history of tobacco and alcohol, history of liver disease and low immunity were the risk factors of drug-induced liver damage. The most common clinical manifestation was hepatocyte damage, 64 cases (52.89%). Conclusion: there are many kinds of drugs that cause drug-induced liver damage. The indication of drug use should be strictly grasped, the combination of drugs should be avoided as far as possible, and the medication scheme should be determined by weighing the advantages and disadvantages of the elderly patients and patients with risk factors.
