【摘要】：目的本研究擬設(shè)計(jì)一個(gè)能夠有效篩檢膽囊息肉病變(PLG)的簡單工具,為PLG的有效管理及防治提供參考。方法采用橫斷面病例-對(duì)照研究設(shè)計(jì)方法,整群抽取2014年1月—8月在陜西省西安市第四軍醫(yī)大學(xué)第一附屬醫(yī)院西京醫(yī)院體檢中心進(jìn)行健康體檢且其體檢信息滿足本次研究要求的12 888名體檢者進(jìn)行問卷調(diào)查、體格檢查、腹部超聲檢查和實(shí)驗(yàn)室檢測(cè)。結(jié)果 12 888名體檢人群中,219例被超聲診斷患有PLG,患病率為1.70%;多因素條件logistic回歸分析結(jié)果表明,在校正了變量間的交互作用及多重共線性后,年齡較大、甘油三酯(TG)較高和間接膽紅素較高是PLG的危險(xiǎn)因素;將這些變量納入構(gòu)建PLG的預(yù)測(cè)模型中,最終構(gòu)建的預(yù)測(cè)指數(shù)為:PLG指數(shù)=13×年齡(年齡≥50,取值為1;年齡50,取值為0)+間接膽紅素+TG;PLG指數(shù)在訓(xùn)練數(shù)據(jù)集中的受試者工作特征曲線(ROC曲線)下面積為0.622(95%CI=0.571~0.673),在驗(yàn)證數(shù)據(jù)集中的ROC曲線下面積為0.615(95%CI=0.562~0.660);PLG指數(shù)P25、P25~P75、P75的PLG的陽性率分別為1.0%、1.7%、2.0%,隨著PLG指數(shù)的增加,PLG的陽性率隨之增加(P0.001)。結(jié)論 PLG指數(shù)在增加篩檢成本效益方面具有重要意義,也可為PLG的有效防治和進(jìn)一步的生活方式治療提供幫助。
[Abstract]:Objective to design a simple tool for the screening of (PLG) in gallbladder polyps, and to provide reference for the effective management and prevention of PLG. Methods A cross-sectional case-control study was used. From January to August 2014, 12 888 medical examiners were selected from Xijing Hospital, the first affiliated Hospital of Xi'an military Medical University, Shaanxi Province, and their physical examination information met the requirements of this study.
