高分辨率顯微內(nèi)鏡對(duì)消化道正常黏膜的成像研究

【摘要】:目的總結(jié)消化道正常黏膜的高分辨率顯微內(nèi)鏡(HRME)的圖片特點(diǎn)。方法選擇正常食道黏膜、胃黏膜、十二指腸黏膜、小腸黏膜和結(jié)腸黏膜活檢標(biāo)本各10例為研究對(duì)象,利用HRME分別對(duì)其進(jìn)行成像觀(guān)察,根據(jù)采集的HRME圖片對(duì)不同部位的消化道正常黏膜進(jìn)行描述性分析,總結(jié)其HRME圖片特點(diǎn)。成像結(jié)束后所有標(biāo)本常規(guī)行病理檢查。結(jié)果 50例活檢標(biāo)本共獲得1 284張HRME圖片,經(jīng)篩查后,留取400張圖片用于結(jié)果分析。不同部位消化道正常黏膜的HRME圖片特點(diǎn)如下:1食道黏膜:細(xì)胞排列規(guī)則,細(xì)胞核圓而亮,大小一致,核間距正常,每個(gè)視野里細(xì)胞的數(shù)量基本一致;2胃底黏膜:淺層可見(jiàn)腺體排列規(guī)則,呈分枝狀。胃小凹開(kāi)口呈圓形或類(lèi)橢圓形,周邊裂隙呈線(xiàn)樣,細(xì)胞核排列規(guī)則;3胃竇黏膜:淺層可見(jiàn)胃小凹開(kāi)口呈不規(guī)則形或管狀,腺腔呈裂隙狀,小凹周?chē)?xì)胞排列規(guī)則,細(xì)胞核小且分布密集;4十二指腸黏膜:可見(jiàn)絨毛呈寬大的指狀,絨毛兩側(cè)有鋸齒狀凹陷,立體感明顯,呈簇狀排列,間隙呈裂隙狀;5小腸黏膜:絨毛狀結(jié)構(gòu)較寬,間隙較寬大,數(shù)量較十二指腸少;6結(jié)腸黏膜:細(xì)胞核大小一致、形態(tài)規(guī)則,為圓形或橢圓形以及菊花狀的腺體結(jié)構(gòu)。所有標(biāo)本病理結(jié)果均證實(shí)為正常黏膜。結(jié)論 HRME可以準(zhǔn)確地辨別不同部位消化道正常黏膜,與病理結(jié)果比較,具有很高的一致性。
[Abstract]:Objective to summarize the image features of high-resolution endoscopic (HRME) of normal digestive tract mucosa. Methods biopsy specimens of normal esophagus mucosa, gastric mucosa, duodenal mucosa, small intestine mucosa and colon mucous membrane were selected as the study objects. HRME was used to observe the pathological changes of esophageal mucosa, gastric mucosa, duodenal mucosa, small intestine mucosa and colon mucosa respectively. According to the collected HRME images, the normal mucosa of digestive tract in different parts were analyzed and the characteristics of HRME images were summarized. Pathological examination was performed on all specimens after imaging. Results A total of 1 284 HRME images were obtained from 50 biopsy specimens. The characteristics of HRME images of normal mucosa of digestive tract were as follows: (1) esophageal mucosa: regular arrangement of cells, round and bright nucleus, uniform size, normal nuclear spacing. The number of cells in each field was basically the same; 2 gastric fundus mucosa: the superficial glands arranged regularly and branched. The opening of gastric fovea was round or ellipsoid, the peripheral fissure was linear, and the nuclei were arranged regularly. 3 gastric antral mucosa: the opening of gastric fovea was irregular or tubular in superficial layer, the glandular cavity was fissure, and the cells around the pit were arranged regularly. The nuclei were small and densely distributed; (4) duodenal mucosa: the villi showed broad finger shape, serrated indentation on both sides of the villi, obvious stereoscopically arranged in clusters and fissures in the interspace; (5) small intestinal mucosa: the villous structure was wider, and the gap was wider. The number of colonic mucosa was less than that of duodenum. 6 colonic mucosa: the nucleus was uniform in size and regular in shape with round or elliptic and chrysanthemum-like glandular structure. All specimens were confirmed as normal mucosa. Conclusion HRME can accurately distinguish normal mucous membrane in different parts of digestive tract, which is consistent with pathological results.
