【摘要】：目的探討內(nèi)鏡下應(yīng)用IT刀聯(lián)合鈦夾和透明帽治療結(jié)直腸帶蒂大息肉的有效性和安全性。方法在透明帽輔助下,用鈦夾夾閉大息肉蒂部阻斷其血供,后用IT刀電切蒂部以切除息肉。通過(guò)回顧2014年1月-2015年12月在中國(guó)人民解放軍第一七五醫(yī)院消化內(nèi)科運(yùn)用此方法治療結(jié)直腸帶蒂大息肉(息肉直徑≥2.0 cm)的45例臨床資料,統(tǒng)計(jì)治療操作時(shí)間、所用鈦夾數(shù)量和并發(fā)癥發(fā)生情況等,分析應(yīng)用此方法切除大息肉的治療效果。結(jié)果 45枚大息肉均一次切除,其中有3例患者創(chuàng)面出現(xiàn)少量滲血,其余術(shù)中無(wú)出血、穿孔;術(shù)后均無(wú)遲發(fā)性出血及穿孔并發(fā)癥發(fā)生。平均使用鈦夾2.7枚,操作時(shí)間3~12 min,平均(5.7±1.2)min。結(jié)論聯(lián)合應(yīng)用IT刀、鈦夾、透明帽治療結(jié)直腸帶蒂大息肉安全、有效,操作較為便捷省時(shí)。
