【摘要】：目的探討不同病理類型結(jié)直腸息肉在無(wú)癥狀人群中的檢出情況及相關(guān)危險(xiǎn)因素。方法收集我院2013-2016年接受結(jié)直腸鏡檢查但無(wú)臨床癥狀的體檢者702例的臨床資料[年齡、性別、體質(zhì)量指數(shù)(BMI)、吸煙情況、一級(jí)親屬結(jié)直腸癌家族史、2型糖尿病及膽囊結(jié)石病史]和腸鏡結(jié)果。根據(jù)腸鏡結(jié)果和息肉病理類型分組,分析不同病理類型息肉的檢出情況及危險(xiǎn)因素。結(jié)果腺瘤、增生性息肉(hyperplasic polyp,HP)、廣基鋸齒狀腺瘤/息肉、傳統(tǒng)鋸齒狀腺瘤、炎性息肉的檢出率分別為14.0%(98/702)、8.8%(62/702)、0.6%(4/702)、0.1%(1/702)、0.9%(6/702),進(jìn)展期腺瘤的檢出率為2.7%(19/702)。年齡≥50歲(50~60歲:OR=1.712,95%CI=1.048~2.798,P=0.032;≥60歲:OR=2.151,95%CI=1.146~4.036,P=0.017)、吸煙≥400支/年(OR=1.866,95%CI=1.120~3.109,P=0.017)、一級(jí)親屬結(jié)直腸癌家族史(OR=1.853,95%CI=1.038~3.310,P=0.037)是腺瘤的獨(dú)立危險(xiǎn)因素;BMI≥24.0 kg/m~2(OR=3.150,95%CI=1.069~9.280,P=0.037)、2型糖尿病(OR=4.430,95%CI=1.248~15.721,P=0.021)是進(jìn)展期腺瘤的獨(dú)立危險(xiǎn)因素;吸煙≥400支/年(OR=2.200,95%CI=1.203~4.024,P=0.010)是HP唯一的獨(dú)立危險(xiǎn)因素。結(jié)論腺瘤及HP是無(wú)癥狀人群結(jié)直腸息肉的主要病理類型。吸煙、50歲以上、一級(jí)親屬結(jié)直腸癌家族史是腺瘤的危險(xiǎn)因素,若同時(shí)合并2型糖尿病或體質(zhì)量超重則增加進(jìn)展期腺瘤的風(fēng)險(xiǎn)。吸煙也是HP的危險(xiǎn)因素,且對(duì)HP的促進(jìn)作用可能更顯著。
[Abstract]:Objective to investigate the detection and risk factors of colorectal polyps in asymptomatic population. Methods the clinical data of 702 patients who underwent colonoscopy but without clinical symptoms in our hospital from 2013 to 2016 were collected [age, sex, body mass index (BMI),) smoking status]. Familial history of first degree relatives with type 2 diabetes mellitus and gallstone. According to the results of colonoscopy and pathological types of polyps, the detection and risk factors of polyps of different pathological types were analyzed. Results the detection rates of adenomas, hyperplasic polyps, wide-base serrated adenomas / polyps, traditional serrated adenomas and inflammatory polyps were 14.0% (98 / 702) and 0.8% (62 / 702) respectively.
