【摘要】：重癥急性胰腺炎(severe acute pancreatitis,SAP)是臨床上常見的一種急危重癥,目前臨床中普遍認(rèn)為高能量消耗、高分解代謝、嚴(yán)重的負(fù)氮平衡導(dǎo)致的營養(yǎng)不良對SAP患者的預(yù)后極其不利。因此營養(yǎng)支持成為治療SAP最重要的手段之一,對SAP的轉(zhuǎn)歸有重要影響。研究證實腸內(nèi)營養(yǎng)(enteral nutrition,EN)與全腸外營養(yǎng)(total parenteral nutrition,TPN)相比可降低重癥急性胰腺炎患者的病死率,減少感染并發(fā)癥及器官衰竭的發(fā)生率。其中,EN中的經(jīng)口進食不僅能減輕患者導(dǎo)管輸入營養(yǎng)的痛苦,減少操作并發(fā)癥,同時可能對胰腺存在保護作用,具有較好的應(yīng)用前景和臨床實用價值。本文就早期經(jīng)口進食進行腸內(nèi)營養(yǎng)在治療SAP的安全性及有效性做一綜述。
[Abstract]:Severe acute pancreatitis (severe acute) is a common acute and critical disease. At present, it is generally considered that high energy consumption, high catabolism and severe negative nitrogen balance caused by malnutrition are extremely unfavorable to the prognosis of SAP patients. Therefore, nutritional support has become one of the most important methods in the treatment of SAP and has an important effect on the outcome of SAP. It has been demonstrated that enteral nutrition (en) can reduce the mortality rate and the incidence of infection and organ failure in patients with severe acute pancreatitis compared with total parenteral nutrition (total parenteral nutation en). The oral feeding in en can not only alleviate the pain of catheter feeding, reduce the complications of operation, but also protect the pancreas, which has a good application prospect and clinical value. This article reviews the safety and efficacy of early oral enteral nutrition in the treatment of SAP.
