【摘要】：目的分析80歲以上老年患者內(nèi)鏡黏膜下剝離術(shù)(ESD)的臨床特點。方法回顧性分析陸軍總醫(yī)院消化內(nèi)鏡中心從2010年9月-2017年1月行ESD治療的80歲以上患者資料,41例患者共行47次ESD。對病變部位、病理類型和并發(fā)癥進行分析。結(jié)果所有患者順利完成ESD術(shù),食管病變3例,胃病變21例,結(jié)腸病變10例,直腸病變13例。術(shù)后病理學(xué)檢查早癌及癌前病變34例,間質(zhì)瘤2例,神經(jīng)內(nèi)分泌腫瘤1例,其他病理類型10例。1例賁門早癌患者追加腹腔鏡下淋巴結(jié)清掃手術(shù)。術(shù)后并發(fā)癥7例,4例為術(shù)后延遲出血,2例為術(shù)后譫妄,1例為肺部感染導(dǎo)致的發(fā)熱。結(jié)論 80歲以上患者結(jié)腸ESD比例高,術(shù)后出血為最主要并發(fā)癥。
[Abstract]:Objective to analyze the clinical features of endoscopic submucosal dissection (ESD) in elderly patients over 80 years old. Methods the data of 41 patients over 80 years old treated with ESD from September 2010 to January 2017 were analyzed retrospectively. The location, pathological type and complications were analyzed. Results all the patients underwent ESD successfully, including esophageal lesions in 3 cases, gastric lesions in 21 cases, colon lesions in 10 cases and rectal lesions in 13 cases. Postoperative pathological examination included early carcinoma and precancerous lesions (34 cases), stromal tumors (2 cases), neuroendocrine tumors (1 case) and other pathological types (10 cases). Postoperative complications in 7 cases were delayed hemorrhage in 4 cases, 2 cases were postoperative delirium and 1 case was fever caused by pulmonary infection. Conclusion the proportion of colon ESD is high in patients over 80 years old, bleeding is the main complication.
