【摘要】：目的觀察烏司他丁聯(lián)合血塞通注射液治療老年急性胰腺炎(AP)患者的療效及對血清白細胞介素(IL)-2、腫瘤壞死因子(TNF)-α及可溶性細胞黏附因子(s ICAM)-1水平的影響。方法 80例老年AP患者隨機分成對照組和試驗組各40例。對照組給予烏司他丁治療,試驗組給予烏司他丁聯(lián)合血塞通注射液治療,兩組接收治療2 w。評價并比較兩組臨床療效。檢測并比較兩組患者治療前后血淀粉酶(BAMY)、尿淀粉酶(UAMY)水平及血清IL-2、TNF-α及sICAM-1水平。記錄并比較兩組患者治療過程中出現(xiàn)的過敏、皮疹、白細胞下降等不良情況發(fā)生率。結(jié)果試驗組總有效率明顯高于對照組(χ~2=4.114,P=0.043)。治療前,兩組BAMY、UAMY水平及血清IL-2、TNF-α、s ICAM-1水平比較差異無統(tǒng)計學(xué)意義(P0.05);治療后,兩組均明顯低于治療前,且試驗組均明顯低于對照組(P0.05)。兩組不良反應(yīng)發(fā)生率比較差異無統(tǒng)計學(xué)意義(χ~2=0.213,P=0.644)。結(jié)論烏司他丁聯(lián)合血塞通注射液治療老年AP的臨床療效顯著,能夠明顯降低BAMY、UAMY水平及血清IL-2、TNF-α、s ICAM-1水平,且安全性較好。
[Abstract]:Objective to observe the effect of ulinastatin combined with Xuesaitong injection on the serum levels of interleukin (IL)-2, tumor necrosis factor (TNF)-偽 and soluble cell adhesion factor (s ICAM)-1 in elderly patients with acute pancreatitis (AP). Methods 80 elderly patients with AP were randomly divided into control group and experimental group, 40 cases in each group. The control group was treated with ulinastatin, the experimental group was treated with ulinastatin combined with Xuesaitong injection, and the two groups were treated for 2 weeks. To evaluate and compare the clinical efficacy of the two groups. The levels of serum amylase (BAMY), urine amylase (UAMY) and serum IL-2 TNF- 偽 and sICAM-1 were measured and compared before and after treatment. The incidence of allergy, rash and leukopenia were recorded and compared between the two groups. Results the total effective rate in the trial group was significantly higher than that in the control group (蠂 2 + 4.114 P < 0.043). Before treatment, there was no significant difference in the levels of BAMYUAMY and serum IL-2TNF- 偽 ICAM-1 between the two groups (P0.05); after treatment, the two groups were significantly lower than before treatment, and the experimental group were significantly lower than the control group (P0.05). There was no significant difference in the incidence of adverse reactions between the two groups (蠂 ~ (2 +) 0.213 P ~ (0.644). Conclusion Ulinastatin combined with Xuesaitong injection is effective in the treatment of senile AP. It can significantly reduce the level of BAMYN UAMY and serum IL-2TNF- 偽 ICAM-1, and is safe.
