【摘要】：目的探討急性胰腺炎患者血漿中性粒細(xì)胞明膠酶相關(guān)脂質(zhì)運(yùn)載蛋白(NGAL)水平變化及臨床意義。方法選取60例急性胰腺炎患者為觀察組,其中輕癥急性胰腺炎(MAP)28例、重癥急性胰腺炎(SAP)32例;常規(guī)治療14天,治療有效48例,治療無效12例(均為SAP)。選取同期健康體檢者35例為對(duì)照組。檢測(cè)觀察組及對(duì)照組血漿NGAL水平(ELISA法檢測(cè))及中性粒細(xì)胞NF-κB mRNA相對(duì)表達(dá)量(RT-PCR法檢測(cè)),分析急性胰腺炎患者NGAL水平與NF-κB mRNA表達(dá)的相關(guān)性。結(jié)果觀察組血漿NGAL水平及中性粒細(xì)胞NF-κB mRNA相對(duì)表達(dá)量均高于對(duì)照組(P均0.01),且SAP患者均高于MAP患者(P均0.05)。治療無效者入院時(shí)血漿NGAL水平及中性粒細(xì)胞NF-κB mRNA相對(duì)表達(dá)量均高于有效者(P均0.01)。急性胰腺炎患者NGAL活性與NF-κB mRNA表達(dá)呈正相關(guān)(r=0.156,P0.05)。結(jié)論急性胰腺炎患者血漿NGAL水平升高,其水平變化參與急性胰腺炎的發(fā)生、發(fā)展,激活NF-κB通路、促進(jìn)炎癥反應(yīng)可能是其作用機(jī)制。
[Abstract]:Objective to investigate the changes and clinical significance of plasma neutrophil gelatinase-associated lipid carrier protein (NGAL) in patients with acute pancreatitis. Methods Sixty patients with acute pancreatitis were selected as the observation group, including 28 patients with mild acute pancreatitis (map) and 32 patients with severe acute pancreatitis (SAP). 35 healthy persons in the same period were selected as the control group. The plasma NGAL level (Elisa) and the relative expression of NF- 魏 B mRNA in neutrophils (RT-PCR) were detected in the observation group and control group. The correlation between NGAL level and NF- 魏 B mRNA expression in acute pancreatitis was analyzed. Results the plasma NGAL level and the relative expression of NF- 魏 B mRNA in neutrophils in the observation group were higher than those in the control group (P0.01), and the levels of SAP patients were higher than those in the map patients (P 0.05). The plasma NGAL level and the relative expression of NF- 魏 B mRNA in neutrophils were significantly higher in the ineffective patients than in the effective ones (P 0.01). There was a positive correlation between NGAL activity and NF- 魏 B mRNA expression in patients with acute pancreatitis (P 0.05). Conclusion the plasma NGAL level in patients with acute pancreatitis is increased. The changes of NGAL level may be involved in the occurrence and development of acute pancreatitis, activation of NF- 魏 B pathway and promotion of inflammatory response.
