【摘要】：目的：了解缺血性肝炎(Ischemic hepatitis，IH)的臨床特點(diǎn)并與急性乙型病毒性肝炎比較，，提高IH的診斷及鑒別診斷水平，降低IH的誤診率。 方法：采用回顧性分析方法收集整理廣西醫(yī)科大學(xué)第一附屬醫(yī)院2013年1月-2014年1月期間的IH58例及2010-2012年期間急性乙型病毒性肝炎32例患者的住院資料，包括性別、年齡、住院時(shí)間、入院前全身基礎(chǔ)病變情況、發(fā)病時(shí)臨床表現(xiàn)、后期的并發(fā)癥情況、住院期間治療情況、預(yù)后情況及實(shí)驗(yàn)室檢查指標(biāo)。運(yùn)用統(tǒng)計(jì)學(xué)方法分析IH的臨床特征并與急性乙型病毒性肝炎進(jìn)行比較。 結(jié)果：58例IH的發(fā)病年齡范圍為1-83歲，平均年齡(48.19士22.74)歲，年齡在45歲以上的有38例，占65.51%。男性44例，女性14例(男:女=3.1:1)。IH臨床表現(xiàn)除了食欲減退、惡心、嘔吐、乏力等癥狀及黃疸、肝腫大的體征，還伴隨有低血壓，呼吸困難，意識(shí)改變、少尿或無(wú)尿、水腫、頸靜脈充盈等表現(xiàn)；肝功能各項(xiàng)指標(biāo)的最高值與正常值上限的倍數(shù)關(guān)系為：丙氨酸氨基轉(zhuǎn)移酶(ALT)：170倍，天門冬氨酸氨基轉(zhuǎn)移酶(AST):441倍，膽紅素(TBIL):10.5倍，堿性磷酸酶(ALP)：13倍，谷氨酰轉(zhuǎn)肽酶(GGT)：14.5，乳酸脫氫酶(LDH)：86倍。伴有多器官功能障礙（MODS）的IH的患者死亡率與無(wú)MODS組的差異有統(tǒng)計(jì)學(xué)意義(P＜0.05)。將IH與急性乙型病毒性肝炎肝功能進(jìn)行比較，兩者TBIL、血清白蛋白（ALB）、血清膽堿酯酶（CHE）、ALP的差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）。 結(jié)論：IH主要在44歲以上的中老年人中發(fā)生；IH臨床表現(xiàn)以原發(fā)基礎(chǔ)疾病的臨床表現(xiàn)為主，肝炎的表現(xiàn)不突出；其肝功能指標(biāo)中血清轉(zhuǎn)氨酶呈現(xiàn)速升速降的特點(diǎn)；IH多數(shù)死于原發(fā)疾病的加重，伴有MODSIH的患者死亡率高于無(wú)MODS的。IH與急性乙型病毒性肝炎比較，肝功能中TBIL、ALP升高不明顯，ALB、CHE下降明顯。
[Abstract]:Objective : To study the clinical characteristics of ischemic hepatitis ( IH ) and compare with acute viral hepatitis B , improve the diagnosis and differential diagnosis of IH , and reduce the misdiagnosis rate of IH .

Methods : A retrospective analysis method was used to collect the data from the first Affiliated Hospital of Guangxi Medical University from January 2013 to January 2014 , including sex , age , hospitalization time , general basal pathological changes before admission , clinical manifestation , late complications , treatment condition , prognosis and laboratory test indexes during hospitalization . The clinical characteristics of IH were analyzed by statistical method and compared with acute viral hepatitis B .

Results : The age of onset of IH was 1 - 83 years , the mean age ( 48.19 鹵 22.74 ) years , the age was 38 cases over 45 years old , accounting for 65.51 % . There were 44 males and 14 females ( male : female = 3.1 : 1 ) . In addition to anorexia , nausea , vomiting , hypodynamia , etc . , the clinical manifestation of IH was accompanied by signs of low blood pressure , dyspnea , consciousness change , oliguria or no urine , edema , jugular vein filling , etc . ;
The relationship between the highest value of liver function indexes and the upper limit of normal value was as follows : alanine aminotransferase ( ALT ) : 170 times , aspartate aminotransferase ( AST ) : 441 times , bilirubin ( TBIL ) : 10.5 times , alkaline phosphatase ( ALP ) : 13 times , glutamyltransferase ( GGT ) : 14.5 , lactate dehydrogenase ( LDH ) : 86 times .

Conclusion : IH occurs mainly in middle - aged and old people over 44 years of age ;
The clinical manifestation of IH is the main clinical manifestation of primary disease , and the manifestation of hepatitis is not prominent .
the serum transaminase in the liver function index is characterized by rapid ascending and descending ;
Most of IH died of primary disease , and the mortality rate of patients with MODSIH was higher than that without MODS . In comparison with acute viral hepatitis B , the increase of TBIL and ALP in liver function was not significant , ALB and CHE decreased .
