【摘要】:目的通過(guò)與肝活檢(金標(biāo)準(zhǔn))對(duì)比,系統(tǒng)評(píng)價(jià)Fibroscan診斷肝纖維化分期的準(zhǔn)確性。方法計(jì)算機(jī)檢索Pubmed、Embase、中國(guó)知網(wǎng)、萬(wàn)方數(shù)據(jù)知識(shí)服務(wù)平臺(tái)及中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)中已發(fā)表的關(guān)于Fibroscan診斷肝纖維化分期的中、英文文獻(xiàn),檢索時(shí)限為2003年1月至2016年1月,對(duì)所選文獻(xiàn)進(jìn)行篩選和評(píng)價(jià),應(yīng)用Stata14.0軟件中雙變量混合效應(yīng)模型進(jìn)行數(shù)據(jù)分析。結(jié)果所得文獻(xiàn)經(jīng)過(guò)去重、閱讀題目、摘要、通讀全文逐步剔除后,最終納入37篇文獻(xiàn),共9167例患者。其中中文文獻(xiàn)4篇,英文文獻(xiàn)33篇,時(shí)間跨度為2005年1月至2015年10月。結(jié)果顯示:(1)Fibro Scan診斷輕微肝纖維化(F≥1)的合并靈敏度(Sen)、特異度(Sep)、陽(yáng)性似然比(PPV)、陰性似然比(NPV)、診斷比值比(OR)及SROC曲線(xiàn)下面積(AUC)分別為0.76、0.80、3.9、0.30、13、0.85;(2)Fibro Scan診斷明顯肝纖維化(F≥2)的Sen、Sep、PPV、NPV、OR及AUC分別為0.77、0.84、4.7、0.28、17、0.88;(3)Fibro Scan診斷嚴(yán)重肝纖維化(F≥3)的Sen、Sep、PPV、NPV、OR及AUC分別為0.84、0.87、6.3、0.19、34、0.92;(4)Fibro Scan診斷早期肝硬化(F≥4)的Sen、Sep、PPV、NPV、OR及AUC分別0.89、0.89、8.2、0.12、67、0.95。結(jié)論 Fibroscan對(duì)肝纖維化分期、尤其是對(duì)嚴(yán)重肝纖維化及早期肝硬化的診斷準(zhǔn)確性較高。
[Abstract]:Objective to evaluate the accuracy of Fibroscan in the diagnosis of hepatic fibrosis stage by comparing with liver biopsy (gold standard). Methods the published Chinese and English literatures published in Pubmedus Embase, China knowledge Network, Wanfang data knowledge Service platform and Chinese Biomedical Literature Database on the diagnosis of hepatic fibrosis staging by Fibroscan were searched by computer. The retrieval time was from January 2003 to January 2016. The selected literatures were screened and evaluated, and the data were analyzed by using the two-variable mixed effect model in Stata14.0 software. Results 9167 patients were included in 37 articles after removing the weight, reading title, abstracts, and reading the full text step by step. There are 4 articles in Chinese and 33 in English, covering the period from January 2005 to October 2015. The results showed that the combined sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, diagnostic ratio and area under the SROC curve were 0.760.803.90.30130.85 Scan for diagnosis of liver fibrosis (F 鈮,
