【摘要】：目的:系統(tǒng)評價α-硫辛酸治療糖尿病胃輕癱患者的臨床療效,分析該藥物治療后的各項(xiàng)癥狀改善及不良反應(yīng)事件發(fā)生情況,為臨床應(yīng)用提供依據(jù)。方法:1檢索Medline、Pubmed、Embase、Cochrane中國知網(wǎng)、萬方和維普等數(shù)據(jù)庫,收集近5年(2011.01.01—2016.12.31)有關(guān)α-硫辛酸治療糖尿病胃輕癱的臨床研究文獻(xiàn)。2應(yīng)用Rev.Man 5軟件,對于α-硫辛酸治療糖尿病胃輕癱的臨床療效(總有效率,胃排空時間、胃排空率,上腹飽脹、噯氣、惡心/嘔吐、食欲不振等癥狀)及血清胃動素和胃泌素水平變化進(jìn)行Meta分析。對于計(jì)量資料選用均數(shù)差值(Mean difference,MD)作為效應(yīng)量。對于計(jì)數(shù)資料選用比值比(Odd Risk,OR)作為效應(yīng)量。結(jié)果:1共獲得符合標(biāo)準(zhǔn)的研究10項(xiàng),共納入研究對象762例(試驗(yàn)組386例,對照組376例),其中涉及總有效率10篇,胃排空時間2篇,胃排空率4篇,上腹飽脹2篇,噯氣2篇,惡心/嘔吐2篇,食欲不振2篇,血清胃動素4篇,血清胃泌素水平3篇,不良反應(yīng)6篇;2 Meta分析結(jié)果顯示:(1)在總有效率(OR=3.76,95%CI=2.54~5.57)、胃排空時間(MD=-1.40,95%CI=-1.88~-0.91)、胃排空率(MD=13.37,95%CI=10.18~16.55)、噯氣(MD=-0.23,95%CI=-0.35~-0.11)、食欲不振(MD=-0.37,95%CI=-0.52~-0.22)等方面療效明顯優(yōu)于對照組;(2)血清胃動素(MD=-125.42,95%CI=-138.79~-122.05)、血清胃泌素(MD=-18.97,95%CI=-26.96~-10.98)水平與對照組比較有明顯差異;(3)在發(fā)生不良反應(yīng)事件的比例(OR=1.82,95%CI=0.74~4.48)、上腹飽脹(MD=-0.26,95%CI=-0.56~0.04)和惡心/嘔吐(MD=-0.11,95%CI=-0.44~0.22)等指標(biāo)方面與對照組無明顯差異。結(jié)論:1α-硫辛酸聯(lián)合促動力藥治療糖尿病胃輕癱具有顯著的臨床療效,可明顯提高DGP患者的胃排空率,縮短胃排空時間,顯著改善患者噯氣、食欲不振等癥狀;療效明顯優(yōu)于單獨(dú)應(yīng)用促動力藥物。2α-硫辛酸可明顯增強(qiáng)促動力藥物治療DGP的臨床療效,可能與其通過抗氧化應(yīng)激損傷,發(fā)揮對胃腸神經(jīng)及胃Cajal間質(zhì)細(xì)胞、平滑肌細(xì)胞的保護(hù)功能,同時改善胃腸激素分泌紊亂,進(jìn)而改善胃動力,促進(jìn)胃排空有關(guān)。3α-硫辛酸治療糖尿病胃輕癱,有良好安全性。4由于本研究納入的文獻(xiàn)質(zhì)量和數(shù)量所限,以上結(jié)論仍需開展更多大樣本、多中心、設(shè)計(jì)嚴(yán)格的隨機(jī)雙盲對照研究進(jìn)一步驗(yàn)證。
[Abstract]:Objective: to evaluate the clinical efficacy of 偽 -lipoic acid in the treatment of diabetic gastroparesis, and to analyze the improvement of symptoms and the occurrence of adverse events after the treatment, so as to provide the basis for clinical application. Methods Rev.Man 5 software was used to search the database of Medlinea Pubmedus Embase Cochrane Cochrane, Wanfang and Wiper in the past 5 years. The clinical literatures of 偽 -lipoic acid on the treatment of diabetic gastroparesis were collected from January 1, 2011 to June 12, 31. 1) the clinical literature of 偽 -lipoic acid in the treatment of diabetic gastroparesis was analyzed by Rev.Man 5 software. Clinical efficacy of 偽 -lipoic acid in the treatment of diabetic gastroparesis (total effective rate, gastric emptying time, gastric emptying rate, upper abdominal fullness, belching, nausea / vomiting, Changes of serum motilin and gastrin levels were analyzed by Meta. For the measurement data, the mean difference (mean difference) is chosen as the effect quantity. For the counting data, the ratio ratio of Odd Riskor was chosen as the effect quantity. Results A total of 10 studies were carried out according to the criteria. 762 subjects (386 in the experimental group and 376 in the control group) were included in the study.
