【摘要】：目的探討腸易激綜合征(IBS)患者腸黏膜肥大細胞(MC)及血管活性腸肽(VIP)的表達及其可能作用。方法 33例腹瀉型IBS患者(D-IBS組)、27例便秘型IBS患者(C-IBS組)及18例健康體檢者(對照組)納入研究。分別取回盲部、乙狀結(jié)腸黏膜行VIP免疫組織化學染色和甲苯胺藍染色MC計數(shù),檢測肛管直腸壓力。結(jié)果 1D-IBS組和C-IBS組乙狀結(jié)腸部MC計數(shù)均較回盲部顯著降低(P0.05),兩組間各部MC計數(shù)比較無統(tǒng)計學差異(P0.05),但各部位均較對照組顯著增高(P0.05)。2D-IBS組和C-IBS組回盲部和乙狀結(jié)腸黏膜VIP表達無統(tǒng)計學差異(P0.05),但各部位均較對照組顯著增高(P0.05)。3三組間靜息壓、最大收縮壓及松弛壓均無統(tǒng)計學差異(P0.05)。D-IBS組和C-IBS組患者感覺閾值、最大容量閾值及疼痛閾值均無統(tǒng)計學差異(P0.05),但與對照組比較均顯著降低(P0.05)。結(jié)論 IBS患者結(jié)腸黏膜MC和VIP表達增加,可能對IBS患者內(nèi)臟高敏感性的形成有一定影響。
[Abstract]:Objective to investigate the expression and possible role of mast cell (MC) and vasoactive intestinal peptide (VIP) in intestinal mucosal mast cells in patients with irritable bowel syndrome (IBS). Methods Twenty-seven patients with constipated IBS (n = 27) and 18 healthy controls (n = 18) were enrolled in the study in D-IBS group (n = 33) and control group (n = 18). VIP immunohistochemical staining and toluidine blue staining were performed to measure the anorectal pressure. Results the count of MC in sigmoid colon in 1D-IBS group and C-IBS group was significantly lower than that in ileocecal part. There was no significant difference in MC count between the two groups. However, there was a significant increase in the ileocecal part and sigmoid mucocolon between 1D-IBS group and C-IBS group. 2D-IBS group and C-IBS group were significantly higher than the control group in ileocecal part and sigmoid colonic mucocolon count. There was no significant difference in the expression of VIP between the three groups, but the resting pressure of the three groups was significantly higher than that of the control group. There was no significant difference in maximum systolic blood pressure and relaxation pressure between P0.05 and D-IBS groups and C-IBS group. There was no significant difference in the threshold of sensation, maximum volume and pain between the two groups (P 0.05), but it was significantly lower than that in the control group. Conclusion the increased expression of MC and VIP in colon mucosa of IBS patients may have an effect on the formation of visceral hypersensitivity in IBS patients.
