脈沖與連續(xù)高強(qiáng)度聚焦超聲對(duì)離體牛肝表面消融的效果比較

【摘要】:目的比較脈沖高強(qiáng)度聚焦超聲(pulsed high-intensity focused ultrasound,PHIFU)與連續(xù)高強(qiáng)度聚焦超聲(continuous high-intensity focused ultrasound,CHIFU)表面消融模式的消融效果,探討優(yōu)化HIFU表面消融模式的輻照策略。方法將40塊牛肝組織按輻照方式不同分為A組(30塊,PHIFU表面消融模式)和B組(10塊,CHIFU表面消融模式),A組再按占空比均分為A1(10%)、A2(30%)、A3(50%)3個(gè)亞組,每個(gè)亞組10塊。B超監(jiān)控下,A、B組分別采用PHIFU與CHIFU表面消融模式對(duì)靶區(qū)進(jìn)行輻照,輻照同時(shí)記錄靶區(qū)內(nèi)部溫度,輻照后觀察靶區(qū)邊緣及其內(nèi)、外未輻照區(qū)損傷情況,測(cè)算能效因子并分析溫度時(shí)間曲線。結(jié)果能效因子:A1組A2組A3組B組。靶區(qū)外部:僅B組出現(xiàn)明顯熱損傷;靶區(qū)邊緣:各組均可形成封閉隔離帶,A1組呈液性壞死,其余組呈凝固性壞死;靶區(qū)內(nèi)部:組織損傷程度隨占空比增大而增大。內(nèi)部溫度:除A1組,其余組最高溫度均超過(guò)60℃。結(jié)論 PHIFU較CHIFU在提高消融效率、減少靶區(qū)外組織損傷上更有優(yōu)勢(shì)。PHIFU表面消融時(shí),調(diào)整占空比可改變靶組織內(nèi)部未輻照區(qū)的溫升及損傷情況。
[Abstract]:Objective to compare the ablation effects of pulsed high-intensity focused ultrasound (PHIFU) and continuous high-intensity focused ultrasound (CHIFUU) surface ablation mode, and to explore the irradiation strategies for optimizing the HIFU surface ablation mode. Methods 40 bovine liver tissues were divided into three subgroups: group A (30 pieces of PHIFU surface ablation model) and group B (10 pieces of CHIFU surface ablation model) and group A were further divided into three subgroups according to their duty cycle. PHIFU and CHIFU surface ablation patterns were used to irradiate the target area respectively. The internal temperature of the target was recorded simultaneously. After irradiation, the damage of the target edge, its inner and outer unirradiated area was observed. Calculate the energy efficiency factor and analyze the temperature-time curve. Results Energy efficiency factor: a 2 group A 1 group A 3 group A 3 group B. Outside the target area: only group B had obvious thermal injury; the edge of target area: all groups could form liquid necrosis in group A1 and coagulation necrosis in group A 1; inside the target area, the degree of tissue damage increased with the increase of duty cycle.
