Hp感染萎縮性胃炎胃黏膜中T淋巴細(xì)胞亞群及PD-1表達(dá)的意義

【摘要】:目的檢測人Hp感染的慢性胃炎(淺表性胃炎、萎縮性胃炎)T淋巴細(xì)胞亞群及PD-1的表達(dá),探討人Hp感染的慢性胃炎(淺表性胃炎、萎縮性胃炎)胃黏膜免疫損傷機(jī)制,為臨床應(yīng)用提供依據(jù)。方法經(jīng)過解放軍第九七醫(yī)院倫理委員會(huì)審批,患者簽署知情同意書。收集胃鏡下鉗取40例Hp陽性胃炎(Hp陽性淺表性胃炎20例、Hp陽性萎縮性胃炎20例),40例Hp陰性胃炎(Hp陰性淺表性胃炎20例、Hp陰性萎縮性胃炎20例)胃黏膜組織標(biāo)本。采用免疫組織化學(xué)方法檢測CD4+T細(xì)胞、CD8+T細(xì)胞、Foxp3、IL-17及PD-1的表達(dá)。Hp陰性、Hp陽性根據(jù)14C呼氣試驗(yàn)和HE染色進(jìn)行判斷,雙陽性為陽性組、雙陰性為陰性組。通過Image-pro plus圖像分析系統(tǒng)測定檢測指標(biāo)累積光密度值(IOD)。結(jié)果1.Hp陽性萎縮性胃炎胃黏膜組中CD4+T細(xì)胞、CD8+T細(xì)胞、Foxp3、IL-17及PD-1的表達(dá)高于Hp陰性萎縮性胃炎胃黏膜組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2.Hp陽性萎縮性胃炎組中CD4+T細(xì)胞、CD8+T細(xì)胞、Foxp3、IL-17及PD-1的表達(dá)高于Hp陽性淺表性胃炎組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。3.Hp陽性胃炎(淺表+萎縮)胃黏膜組中CD4+T細(xì)胞、CD8+T細(xì)胞、Foxp3、IL-17及PD-1的表達(dá)量高于Hp陰性胃炎(淺表+萎縮)胃黏膜組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。4.Hp陽性淺表性胃炎胃黏膜組中CD4+T細(xì)胞、IL-17及PD-1的表達(dá)高于Hp陰性淺表性胃炎胃黏膜組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。5.Hp陽性淺表性胃炎胃黏膜組中CD8+T細(xì)胞和Foxp3的表達(dá)高于Hp陰性淺表性胃炎胃黏膜組,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論1.Hp感染陽性胃炎胃黏膜中T淋巴細(xì)胞亞群(CD4+T細(xì)胞、CD8+T細(xì)胞的浸潤和Foxp3、IL-17)及PD-1表達(dá)高于Hp陰性胃炎。2.Hp感染陽性胃炎胃黏膜中,萎縮性胃炎T淋巴細(xì)胞亞群及PD-1表達(dá)高于淺表性胃炎。
[Abstract]:Objective to detect the expression of T lymphocyte subsets and PD-1 in chronic gastritis (superficial gastritis, atrophic gastritis), and to explore the mechanism of gastric mucosal immune damage in chronic gastritis (gastritis, atrophic gastritis) infected with HP.To provide the basis for clinical application.Methods the patients signed informed consent after the examination and approval of the 97 th PLA Hospital Ethics Committee.Gastric mucosa specimens were collected from 40 cases of HP positive gastritis / HP positive superficial gastritis 20 cases of HP positive atrophic gastritis 20 cases of HP negative gastritis / HP negative superficial gastritis 20 cases of HP negative atrophic gastritis and 20 cases of HP negative atrophic gastritis under gastroscope.Immunohistochemical method was used to detect the expression of Foxp3mIL-17 and PD-1 on CD4 T cells. HP negative Hp-positive was determined by 14C breath test and HE staining. The positive group was positive in double positive group and negative group in double negative group.The cumulative optical density was measured by Image-pro plus image analysis system.Results the expression of CD4 T cell and CD8 T cell Foxp3 IL-17 and PD-1 in 1.Hp positive atrophic gastritis group was higher than that in HP negative atrophic gastritis group.There was a significant difference in the expression of Foxp3, IL-17 and PD-1 in CD4 T cells and CD8 T cells in patients with atrophic gastritis with HP positive, and the expression of Foxp3 IL-17 and PD-1 in HP positive atrophic gastritis group was higher than that in HP positive superficial gastritis group.There was a significant difference in the expression of CD4 T cell and CD8 T cell Foxp3IL-17 and PD-1 in gastric mucosa group with HP positive gastritis (superficial atrophy) than in HP negative gastritis (superficial atrophy) group, and the expression of Foxp3IL-17 and PD-1 in HP negative gastritis (superficial atrophy) group was higher than that in HP negative gastritis (superficial atrophy) group.The expression of IL-17 and PD-1 of CD4 T cells in the gastric mucosa of superficial gastritis with HP positive was higher than that in the gastric mucosa with HP negative superficial gastritis, and the expression of IL-17 and PD-1 in gastric mucosa of HP negative superficial gastritis was higher than that of HP negative superficial gastritis.The expression of CD8 T cells and Foxp3 in gastric mucosa of patients with HP positive superficial gastritis was significantly higher than that with HP negative superficial gastritis. There was no significant difference in the expression of CD8 T cells and Foxp3 in gastric mucosa of patients with HP negative superficial gastritis.Conclusion the infiltration of T lymphocyte subsets (CD 4 T cells) and the expression of Foxp3 (IL-17) and PD-1 in gastric mucosa of patients with 1.Hp positive gastritis are higher than those of HP negative gastritis. 2.The expression of T lymphocyte subsets and PD-1 in atrophic gastritis was higher than that in superficial gastritis.
