【摘要】：目的觀察靜脈注射(靜注)ω-3魚油脂肪乳對重癥急性胰腺炎患者全身炎癥反應(yīng)的影響。方法將40例重癥急性胰腺炎患者隨機(jī)分為研究組和對照組,各20例。兩組在給予抗感染,抑制胰酶分泌、胰酶活性,營養(yǎng)支持等治療的基礎(chǔ)上分別靜脈注射ω-3魚油脂肪乳(研究組)和等滲鹽水(對照組),治療7 d后采用全自動生化分析儀和ELISA法檢測兩組CRP、降鈣素原(PCT)、腫瘤壞死因子α(TNF-α)、IL-6水平,并對兩組進(jìn)行急性生理與慢性健康評分(APACHE-Ⅱ評分)。結(jié)果研究組、對照組治療前CRP分別為(130.6±12.8)與(120.2±13.1)mg/L,PCT分別為(20.4±4.0)與(21.2±4.3)μg/L,TNF-α分別為(70.4±18.7)與(69.8±17.6)ng/L,IL-6分別為(295.6±67.7)與(302.4±65.6)μmol/L,APACHE-Ⅱ評分分別為(17.5±6.4)與(17.8±8.2)分。研究組、對照組治療后CRP分別為(34.9±11.5)與(58.4±12.1)mg/L,PCT分別為(5.4±3.5)與(10.2±4.2)μg/L,TNF-α分別為(20.5±15.4)與(42.3±14.5)ng/L,IL-6分別為(201.4±64.5)與(254.5±66.3)μmol/L,APACHE-Ⅱ評分分別為(10.3±4.2)與(14.3±5.7)分,兩組治療前后及治療后兩組間各指標(biāo)比較,P均0.05。結(jié)論靜注ω-3魚油脂肪乳可降低重癥急性胰腺炎患者全身炎癥反應(yīng)程度。
[Abstract]:Objective to observe the effect of intravenously injected 蠅 -3 fish oil fat emulsion on systemic inflammation in patients with severe acute pancreatitis.Methods 40 patients with severe acute pancreatitis were randomly divided into study group and control group with 20 cases each.The two groups were given anti-infection, inhibition of pancreatic enzyme secretion, trypsin activity,On the basis of nutritional support and other treatments, 蠅 -3 fish oil fat emulsion (study group) and isosmotic saline (control group) were injected intravenously respectively.
