【摘要】：目的探討幽門螺桿菌(Hp)感染與Barrett食管發(fā)生發(fā)展的關(guān)系。方法采用計算機檢索和手工檢索相結(jié)合的方法搜索Pubmed、CNKI、OVID、Wiley InterScience、萬方數(shù)據(jù)庫、Embase數(shù)據(jù)庫,時間從建庫到2014年1月,納入分析Hp感染與Barrett食管相關(guān)性的病例對照研究。使用RevMan 5.2及Stata 12.0進(jìn)行Meta分析,采用I2檢驗判斷各研究間的異質(zhì)性,敏感性分析評估其結(jié)果穩(wěn)定性,應(yīng)用Begg′s漏斗圖和Egger′s回歸圖評價發(fā)表偏倚。結(jié)果共納入14篇文獻(xiàn),病例組2 103例,對照組內(nèi)鏡檢查健康者7 009例,胃食管反流性疾病(GERD)患者2 213例。Barrett食管患者的Hp感染率及CagA陽性的Hp感染率,與健康者比較(OR=0.60,95%CI:0.52~0.68;OR=0.24,95%CI:0.11~0.52);Barrett食管患者的Hp感染率及CagA陽性的Hp感染率,與GERD患者比較(OR=0.02,95%CI:0.06~0.02;OR=0.44,95%CI:0.14~1.45)。結(jié)論 Hp尤其是CagA陽性的菌株可能對Barrett食管的發(fā)生有保護作用。
[Abstract]:Objective to investigate the relationship between Helicobacter pylori (HP) infection and the development of Barrett esophagus.Methods the method of computer search and manual search was used to search the Embase database of Pubmedus CNKIIDOVIDE Wiley InterScience. from the time of establishment to January of 2014, a case-control study was carried out to analyze the correlation between HP infection and Barrett esophagus.RevMan 5.2 and Stata 12.0 were used for Meta analysis, I 2 test was used to judge the heterogeneity of each study, sensitivity analysis was used to evaluate the stability of the results, and Begg's funnel diagram and Egger's regression diagram were used to evaluate publication bias.Results there were 2 103 cases in the case group, 7 009 cases in the control group and 2 213 cases in the patients with gastroesophageal reflux disease (GERD). The HP infection rate and CagA positive HP infection rate in the patients with Barrett's esophagus were studied.涓庡仴搴瘋，
