獻(xiàn)血者HBsAg陰性HBV DNA陽(yáng)性血液樣本電化學(xué)發(fā)光檢測(cè)分析

【摘要】:目的對(duì)獻(xiàn)血者血液篩查HBs Ag陰性HBV DNA陽(yáng)性的樣本,用電化學(xué)發(fā)光免疫分析法(ECLIA)和定量核酸(NAT)測(cè)定的結(jié)果并進(jìn)行分析。方法對(duì)本血站2016年3-11月56 448例無償獻(xiàn)血者血液標(biāo)本篩查出97例HBs Ag陰性HBV DNA定性檢測(cè)陽(yáng)性的樣本(實(shí)驗(yàn)組)進(jìn)行HBV DNA定量檢測(cè),再與隨機(jī)抽取的100例HBs Ag和HBV DNA均陰性的樣本(對(duì)照組)分別進(jìn)行ECLIA檢測(cè)HBs Ag和HBc Ab。結(jié)果實(shí)驗(yàn)組NAT定量檢測(cè)HBV陽(yáng)性有55例,其中病毒載量20 IU/m L有36(65.45%)例,病毒載量20 IU/m L有19(34.55%)例,病毒載量范圍在(37.4~394)IU/m L(中位數(shù)41.3 IU/m L),與NAT定性陽(yáng)性符合率為56.70%;實(shí)驗(yàn)組用ECLIA檢出HBs Ag陽(yáng)性15例,而對(duì)照組未檢出HBs Ag陽(yáng)性樣本(χ2校正=14.612,P0.05);實(shí)驗(yàn)組的HBc Ab陽(yáng)性率96.91%(94/97)明顯高于對(duì)照組的38.00%(38/100)(χ2=77.284,P0.05),差異有統(tǒng)計(jì)學(xué)意義。結(jié)論核酸檢測(cè)技術(shù)應(yīng)用于獻(xiàn)血者血液篩查可有效降低隱匿性乙型肝炎的輸血傳染;電化學(xué)發(fā)光免疫分析法能降低ELISA漏檢HBs Ag風(fēng)險(xiǎn);HBc Ab在HBV DNA陽(yáng)性樣本中的陽(yáng)性率較高,可作為血液篩查的補(bǔ)充方法。
[Abstract]:Objective to detect and analyze the results of HBs Ag negative HBV DNA positive blood screening samples from blood donors by electrochemiluminescence immunoassay (ECLIA) and quantitative nucleic acid (Nat) assay.Methods 97 HBs Ag negative HBV DNA samples (experimental group) were screened out from 56,448 volunteer blood donors from March to November 2016. HBV DNA was detected quantitatively in the experimental group.Then ECLIA was used to detect HBs Ag and HBc Abb in 100 randomly selected samples with both HBs Ag and HBV DNA negative (control group).Results in the experimental group, 55 cases were positive for HBV by NAT quantificationally, including 3655.45 cases with virus load of 20 IU/m L and 1934.55 cases with 20 IU/m L virus load.The viral load was in the range of 37.4 渭 mol / mL (median 41.3 IU/m / L), and the qualitative coincidence rate with NAT was 56.70.The ECLIA was used to detect HBs Ag in 15 cases in the experimental group.Conclusion the application of nucleic acid detection in blood screening of blood donors can effectively reduce blood transfusion infection in patients with occult hepatitis B. Electrochemiluminescence immunoassay can reduce the positive rate of HBV DNA in HBV DNA positive samples.It can be used as a supplementary method for blood screening.
