【摘要】：目的觀察并對比乳酸左氧氟沙星/頭孢唑肟在急性水腫型膽源性胰腺炎治療中的效果,為急性胰腺炎的臨床規(guī)范抗感染治療提供參考。方法選取2014年1月至2014年11月期間在本院急診科病區(qū)連續(xù)收治的180例急性水腫型膽源性胰腺炎患者,隨機分為乳酸左氧氟沙星治療組與頭孢唑肟治療組,在禁食水、胃腸減壓、抑酸、抑酶、營養(yǎng)支持等常規(guī)治療基礎上分別選用乳酸左氧氟沙星及頭孢唑肟抗感染,監(jiān)測患者嘔吐、腹痛、腹脹緩解情況及時間,開放飲食時間,住院天數(shù),血常規(guī)、C反應蛋白(CRP)、生化、血清淀粉酶變化及影像學胰腺炎癥吸收情況。結(jié)果乳酸左氧氟沙星治療組總有效率高于頭孢唑肟治療組(P0.05);與頭孢唑肟治療組比較,乳酸左氧氟沙星治療組嘔吐、腹痛、腹脹緩解時間縮短(P0.01),開放飲食時間提前,住院天數(shù)縮短(P0.05),血常規(guī)、CRP及血淀粉酶恢復正常時間均提前(P0.05),影像學胰腺炎癥吸收快(P0.05)。結(jié)論在急性水腫型膽源性胰腺炎抗感染治療中,乳酸左氧氟沙星效果優(yōu)于頭孢唑肟。
[Abstract]:Objective to observe and compare the efficacy of levofloxacin lactate / cefazoxime in the treatment of acute edematous biliary pancreatitis. Methods from January 2014 to November 2014, 180 patients with acute edematous biliary pancreatitis were treated in the emergency department of our hospital from January 2014 to November 2014. Levofloxacin lactate group and cefazoxime group were randomly divided into two groups. Levofloxacin lactate and cefazoxime were used to fight infection on the basis of fasting water, gastrointestinal decompression, acid suppression, enzyme inhibition, nutritional support and so on. Patients with vomiting, abdominal pain, abdominal distension relief and time, open diet time, hospitalization days, blood routine C-reactive protein (CRP), biochemical, Results the total effective rate of levofloxacin lactate treatment group was higher than that of cefoxime treatment group (P 0.05), and compared with cefozoxime treatment group, levofloxacin lactate treatment group had vomiting and abdominal pain. The remission time of abdominal distension was shortened (P 0.01), the time of open diet was shortened, the days of hospitalization was shortened (P 0.05), the time of routine blood routine CRP and serum amylase returned to normal were prematurely (P 0.05), and the absorption of imaging pancreatitis was rapid (P 0.05). Conclusion in the treatment of acute edematous biliary pancreatitis, it is possible to prevent infection. The effect of levofloxacin lactate was better than that of cefazoxime.
